Testis Cancer: When Should You Do Retroperitoneal Dissection after Chemotherapy of Stage lib-Ill Nonseminoma Germ-Cell Tumors?

Surgical resection of postchemotherapy residual retroperitoneal lymph nodes (PC-RPLND) or residual visceral metastatic deposits represents an integral part of the multimodality treatment for patients with advanced testicular cancer. The rationale for PC-RPLND is to remove persistent disease that may contain mature teratoma in approximately 30% to 40% and vital cancer in about 10% to 20% of the patients. In nonseminoma germ-cell tumors (NSGCT), PC-RPLND is only indicated in men with normalized or plateauing serum tumor markers and residual disease.
