Objective:To explore the effect of tiotropium bromide combined with symbieort turbuhaler on respiratory function and exercise tolerance in elderly patients with severe COPD.Methods:A total of 106 patients with severe stable COPD were randomly divided into group A and group B,each group with 53 cases.Group A received inhalation of tiotropium bromide powder (18 μg,1/d) combined with symbicort turbuhaler (160/5.4 μg,2/d),and group B received inhalation of tiotropium bromide powder (18 μg,1/d) alone for 6 months.The pulmonary function,arterial blood gas,IL-8,IL-10 induced in sputum and exercise tolerance of all patients were assessed before and six months after the treatment.Results:Compared with before the treatment,levels of FEV1% pred,FEV1/FVC,6MWD,PaO2 and IL-10 induced in sputum were significantly increased while PaCO2 and IL 8 induced in sputum were significantly decreased in two groups after the treatment.Improvements of all parameters were more obvious in group A than those in group B.Conclusion:The tiotropium bromide combined with symbicort turbuhaler was superior to any single-agent treatment in improving airway inflammation,respiratory function and exercise tolerance in elderly patients with severe stable COPD.%目的:探讨噻托溴铵联合布地奈德/福莫特罗吸入对老年重度慢性阻塞性肺疾病(COPD)患者呼吸功能和运动耐力的影响.方法:106例老年重度COPD稳定期患者随机分为A组和B组,每组53例.A组给予噻托溴铵(18μg,1/d)联合布地奈德/福莫特罗吸入(160/5.4 μg,2/d);B组仅给予噻托溴铵吸入,剂量同A组.治疗前、治疗6个月后测定肺功能、动脉血气、诱导痰IL-8和IL-10及运动耐力.结果:与治疗前比较,两组治疗后FEV1%pred、FEV1/FVC、6MWD、PaO2、诱导痰IL-10明显增加,PaCO2、诱导痰IL-8明显降低,A组较B组改善更明显.结论:噻托溴铵联合布地奈德/福莫特罗吸入能更好地改善老年重度COPD患者的气道炎症、呼吸功能及提高运动耐力.
