DSCT双能量技术检测痛风患者尿酸盐结晶沉积的初步应用

         

摘要

Objective:To estimate the value of dual-energy computed tomography(DECT)technique in detection of uric acid(UA)crystals in joints or periarticular structures in patients with gout.Method:A total of 24 patients underwent DECT of a painful joint and 10 control patients were included. A material decomposition algorithm was used to identify uric acid. Two trained radiologists evaluated the dual-energy CT images and classified the examination findings as positive or negative for the presence of uric acid crystals.Result:DECT scans revealed a total of 223 areas of urate of deposition in 24 patients. Whereas physical examination only showed 88 areas of urate deposition,accounts for about 39.4%(P<0.01)No UA deposit was observed in all 10 control patients.Conclusion:DECT is a sensitive and noninvasive method for identifying uric acid deposits in joints and periarticular soft tissues in patients suspected of having gout.%  目的:评价双源 CT(DSCT)双能量扫描技术在痛风患者四肢关节及周围软组织尿酸盐沉积诊断中的作用.方法:对24例临床诊断痛风的患者进行患病关节的双源 CT 双能量扫描,对照组为10列非痛风患者.采用双能量 CT 物质分解算法处理图像后,由2名医师独立进行评价,根据关节有无尿酸盐结晶沉积将结果判定为阳性或阴性.比较病例组和对照组尿酸盐结晶沉积的差异及双源双能量 CT 检出尿酸盐沉积部位与临床评估的差异.结果:通过双源 CT 双能量扫描在24例痛风患者四肢关节共发现223处尿酸盐结晶沉积,而临床四肢体格检查仅能发现88处关节疑有病变,仅占双源 CT 双能量扫描发现病灶的39.4%,差异具有统计学意义(P=0.000).对照组未发现尿酸盐结晶的沉积.结论:双源 CT 双能量扫描在对痛风疑似患者关节及周围软组织尿酸盐结晶沉积的检测中具有敏感性、特异性和无创性.
