2型糖尿病患者糖化血红蛋白和空腹血糖水平与视网膜病变的相关性研究

         

摘要

目的:探讨2型糖尿病患者糖化血红蛋白(HbA1c)和空腹血糖(FPG)水平与视网膜病变(DR)的相关性。方法:通过眼底检查或眼底血管荧光素造影结果,将106例T2DM患者分为NDR组64例(无视网膜病变组)和DR组42例(视网膜病变组),其中NPDR组31例(非增生性视网膜病变),PDR组11例(增生性视网膜痛变);另选取普通人群50例作为正常对照组。分别测定其HbA1c和FPG水平,所得结果进行统计分析。结果:DR组的HbA1c及FPG水平显著高于NDR组和正常对照组,NDR组显著高于正常对照组,各组比较差异均有统计学意义(P<0.05)。PDR组的HbA1c和FPG水平显著高于NPDR组,且NPDR组的HbA1c水平显著高于NDR组,差异均有统计学意义(P<0.05);但NPDR组与NDR组的FPG水平比较差异无统计学意义(P>0.05)。各组的FPG与HbA1c相关性分析显示,DR组呈明显正相关(r=0.801),NDR组也呈明显正相关(r=0.739)。结论:T2DM患者HbA1c和FPG水平与DR的发生、发展及程度有较显著的相关性,可作为糖尿病眼底病变监控的重要指标。%Objective: To investigate the relationship of glycosylated hemoglobin (HbA1c) and fasting blood glucose (FPG) levels in type 2 diabetes mellitus patients with diabetic retinopathy (DR).Method:Through the results of fundus examination and fundus fluorescein angiography,106 patients with T2DM were divided into NDR group of 64 cases(without retinopathy group) and DR group of 42 cases (diabetic group), NPDR group of 31 cases (non proliferative retinopathy), PDR group of 11 cases (proliferative retinopathy); 50 cases of normal people were selected as the normal control group. The HbA1c and FPG levels were measured, the results were statistically analyzed.Result:Compared the HbA1c and FPG levels: DR group was higher than NDR group and normal control group, the NDR group was significantly higher than the normal control group, the differences were statistically significant(P<0.05). The levels of FPG and HbA1c in PDR group were significantly higher than those in NPDR group, and the level of HbA1c in NPDR group was significantly higher than that in NDR group,the differences were statistically significant(P<0.05), but there was no significant difference between NPDR group and NDR group (P>0.05).Correlation analysis between groups FPG and HbA1c showed:there were significant positive correlation in group DR (r=0.801) and NDR group (r=0.739).Conclusion: There is a significant correlation between the occurrence, development and severity of T2DM in patients’ HbA1c and FPG level with DR,it can be used as an important index of fundus disease monitoring.
