目的：探讨非酒精性脂肪肝（non-alcoholic fatty liverdisease，NAFLD）合并阻塞性睡眠呼吸暂停低通气综合征（obstructive sleep apnea hypopnea syndrome，OSAHS）的患者使用无创呼吸机的临床疗效。方法：选取2013年6月-2015年12月本院确诊为非酒精性脂肪肝合并阻塞性睡眠呼吸暂停低通气综合征的患者68例作为研究对象，随机分为对照组和治疗组，每组34例。对照组给予一般健康指导并口服常规护肝药物治疗，治疗组在对照组基础上给予患者夜间睡眠期间佩戴使用全自动无创呼吸机，每晚使用7~8 h。观察两组患者治疗前后血清丙氨酸氨基转移酶（ALT）及肝脏超声检测变化。结果：治疗3个月后，两组ALT均较治疗前降低，且治疗组低于对照组，比较差异均有统计学意义（P<0.05）；治疗组好转率为81.25%，明显高于对照组的58.82%，比较差异有统计学意义（P<0.05）。结论：临床上在治疗NAFLD合并OSAHS的患者时，除了常规运动、饮食指导及护肝药物治疗外，同时在夜间睡眠时使用无创呼吸机治疗，能取得满意的临床效果，值得推广。%Objective:To investigate the clinical efficacy of non-invasive ventilator in the treatment of non-alcoholic fatty liver disease (NAFLD)and obstructive sleep apnea hypopnea syndrome (OSAHS).Method:From June 2013 to December 2015,68 cases of NAFLD combined with OSAHS were selected as the research objects, they were randomly divided into control group and treatment group,each group had 34 cases.The control group was given general health guidance and took orally conventional liver protective drug,treatment group was used for the full automatic non-invasive breathing machine during the night sleep on basis of the control group,it was used 7 to 8 hours every night.The changes of serum alanine aminotransferase (ALT) and liver ultrasonic testing before and after treatment in two groups were observed.Result:After 3 months treatment,two groups of ALT were lower than before treatment,and the treatment group was lower than control group,the differences were statistically significant(P<0.05).The improvement rate of the treatment group was 81.25%,which was significantly higher than 58.82% of the control group,the difference was statistically significant(P<0.05).Conclusion:Clinically in the treatment of NAFLD patients with OSAHS,not noly regular exercise,diet guidance and liver protection therapy,but also use for the full automatic non-invasive breathing machine during the night sleep can achieve satisfactory clinical results and is worthy of promotion.
