Objective:To compare the clinical efficiency laparoscopic right hemicolectomy with conventional open right hemicolectomy, investigate the safety and effectiveness of the former.Method: 53 right colon carcinoma patients in First Affiliated Hospital of Sun Yat-sen University from January 2010 to January 2013 were selected, among 25 patients who underwent laparoscopic right hemicolectomy(laparoscopy group) and 28 patients who underwent conventional open right hemicolectomy(open group). The operative time, blood loss, numbers of lymph nodes harvested time to first postoperative flatus,days of postoperative stay and postoperative complications of the two groups were compared.Result:Blood loss of the laparoscopy group was significantly less than the open group [(69.8±30.2) vs (133.6±36.6) mL] (P<0.05). The time to first postoperative flatus [(49.7±9.6)vs (60.6±10.7)h] and days of postoperative stay [(7.0±1.4) vs (11.5±2.4)d] in the laparoscopy group were significantly shorter than those in the open group(P<0.05). There were no significant differences in operative time,numbers of lymph nodes harvested and postoperative complications of the two groups (P>0.05).Conclusion:Laparoscopic right hemicolectomy have the advantages of excellent minimally invasive effect, and is also accord with principle of malignant tumor radical resection. So it is safe and effective , be worth to promote.%目的：比较腹腔镜右半结肠切除术和开放右半结肠切除术的临床效果，探讨前者的安全性和有效性。方法：分析2010年1月-2013年1月中山大学附属第一医院胃肠外科收治的53例右半结肠癌患者的临床资料，其中25例行腹腔镜右半结肠切除术，28例行开放右半结肠切除术，比较两组患者的手术时间、术中出血量、清除淋巴结数、术后肛门排气时间、术后住院时间及术后并发症等。结果：腹腔镜组出血量（69.8±30.2）mL少于开腹组的（133.6±36.6）mL，术后肛门排气时间（49.7±9.6）h小于开腹组的（60.6±10.7）h，术后住院时间（7.0±1.4）d短于开腹组的（11.5±2.4）d，两组患者比较差异均有统计学意义（P<0.05）。而两组患者的手术时间、清除淋巴结数及术后并发症发生率比较差异均无统计学意义（P>0.05）。结论：腹腔镜右半结肠切除术具有手术创伤小、疼痛轻、康复快的优点并且符合肿瘤根治原则，是安全有效的，值得推广。
