海南省三甲医院医护人员慢性疲劳与心理健康状况的关系

摘要

目的：了解海南省三甲医院医护人员慢性疲劳现况及其与心理健康状况的关系。方法：采用多阶段随机抽样的方法抽取海南省三甲医院医护人员1042人，采用横断面研究的方法，运用疲劳量表14（ FS-14）和心理症状自评量表（SCL-90）进行测评。结果：①疲劳总平均得分为（7．43±3．04）分，躯体疲劳得分（4．79±2．31）分，脑力疲劳得分（2．63±1．40）分，慢性疲劳的发生率为38．77％（404/1042），有心理疾病倾向者占11．80％（123/1042）。②躯体疲劳、脑力疲劳、疲劳总平均得分与SCL-90各因子分均有线性正相关关系（ r＝0．220～0．422， P＜0．001）。③疲劳的心理因素混合线性模型分析结果显示，躯体化、强迫症状、抑郁、恐怖、偏执、精神病性为医护人员慢性疲劳的影响因素（β＝1．551、1．534、0．888、1．102、0．772、1．311，P均＜0．05）。结论：医护人员慢性疲劳与心理健康水平有关。%Aim:To study the prevalence of chronic fatigue of medical staff from 3A-hospitals in Hainan Province and its relationship with mental health conditions .Methods:By multi-stage random sampling , a total of 1 042 doctors and nur-ses from three 3A-hospitals in Hainan were selected for the cross-section study.All subjects were assessed by Fatigue Scale 14(FS-14)and Mental Symptom Checklist 90(SCL-90).Results:The mean total score of FS-14 was (7.43 ±3.04), the score of physical fatigue was (4.79 ±2.31),and that of mental fatigue was (2.63 ±1.40).The prevalence of chronic fa-tigue was 38.77%(404/1 042),and 11.80%(123/1 042) were in psychological disorder station .Physical fatigue, mental fatigue, and the mean total score were positively correlated with the scores of every factor of SCL-90(r=0.220-0.422, P<0.001).The result of mixed linear model showed that somatization symptoms , obsessive-compulsive symptoms,depres-sion, terror, paranoia, and psychosis would impact fatigue significantly (β=1.551,1.534,0.888,1.102,0.772,1.311, P<0.05).Conclusion:Chronic fatigue status of medical staff is related with the psychological health level .
