支气管镜单向活瓣肺减容术对重度非均质性肺气肿患者多种炎症指标的影响

Objective To investigate the levels of multiple inflammatory markers before and after bronchoscopic one-way valve lung volume reduction surgery(LVRS)in patients with severe heterogeneous emphysema.Methods A total of 48 patients with severe heterogeneous emphysema treated by bronchoscopic one-way valve LVRS were selected in this study.The erythrocyte sedimentation rate(ESR),adiponeclin(ADPN),C-reactive protein(CRP),interleukin-6(IL-6),interleukin-8(IL-8)and procalcitonin(PCT)in blood samples were measured before and after LVRS.Results There was no significant differences in the levels of ESR and ADPN before and after LVRS(P>0.05).The levels of CRP,IL-6,IL-8 and PCT decreased significantly after LVRS compared with those before LVRS(P<0.05).Conclusions Bronchoscopic one-way valve LVRS can reduce the inflammatory response in patients with severe heterogeneous emphysema,and might not increase the risk of infection.%目的 观察支气管镜单向活瓣肺减容术(lung volume reduction surgery,LVRS)对于重度非均质性肺气肿患者多种炎症指标的影响,评估患者行支气管镜单向活瓣LVRS的炎症反应强度.方法 选取经支气管镜单向活瓣LVRS治疗的48例重度非均质性肺气肿患者作为研究对象.检测患者术前及术后红细胞沉降率(ESR)、脂联素(ADPN)、C反应蛋白(CRP)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、降钙素原(PCT)水平.结果 LVRS前后患者ESR、ADPN的水平差异无统计学意义(P>0.05).LVRS后患者血清CRP、IL-6、IL-8和PCT的水平均较LVRS前显著下降(P<0.05).结论 支气管镜单向活瓣LVRS后非均质性肺气肿患者的炎症反应降低,可能并不增加患者感染的风险.
