New one-way valve for bronchoscopic lung volume reduction (BLVR) in patients with advanced emphysema and giant bulla

We developed a silicone one-way valve (Endobronchial Miyazawa Valve; EMV) as the new device for BLVR. We examined the clinical efficacy and safety with EMV in this trial. Methods: 16 patients had advanced emphysema and 3 patients had giant bulla. The efficacy was measured by MRC Scale, Borg Scale, 6-MWT, target lobar volume, and pulmonary functions. Results: MRC Scale, Borg Scale and 6-MWT were improved. The target lobar volume was reduced significantly but the changes of respiratory function tests were various. In complications, no acute and severe exacerbation was observed. Mild obstruction pneumonia occurred in 2 patients, ipsilateral pneu-mothorax in one patient and late migration in 2 patients. In one patient, malfunction of EMV was observed two years later. Conclusions: We considered that BLVR with EMV was effective and safe therapy for advanced emphysema and giant bulla.
