结核病患者外周血CD4+记忆T细胞亚群的检测及意义

         

摘要

目的:探讨结核病患者外周血总CD4+T细胞及抗原特异性记忆T细胞的产生及分布规律,了解记忆T细胞在结核病发病中的作用.方法:以荧光标记的抗CD4、CCR7、CD45RA三种抗体共染色,用流式细胞仪检测结核病患者外周血总CD4+记忆T细胞,以荧光标记的抗人CD4、CD154、CCR7、CD45RA四种抗体共染色,检测结核抗原特异性CD4+记忆T细胞.通过对比分析,了解记忆T细胞分布特征.结果:结核患者外周血总CD4+T细胞中CD45RA+ CCR7+T细胞、CD45RA-CCR7+T细胞和CD45RA-CCR7-T细胞这三群的比例分别为59.18%±16.11%、15.40%±6.48%和16.30%±9.97%;而在抗原特异性CD4+T细胞中三群T细胞的比例分别为32.67%±13.94%、56.96%±12.70%和8.76%±5.62%.通过统计学比较后发现,总CD4+T细胞和抗原特异性CD4+T细胞这两组之间CD45RA+CCR7+T细胞、CD45RA-CCR7+T细胞和CD45RA-CCR7-T细胞的分布比例差异均有显著性.结论:结核病患者外周血总CD4+T细胞中以初始细胞为主,而抗原特异性CD4+T细胞中则以中央记忆细胞为主,说明结核病患者可产生抗原特异性记忆T细胞,参与长效免疫,可能在防止再感染或降低再感染的严重程度方面起重要作用.%Objective To study the production and distribution of total CD4 memory T cells and antigen-specific memory T cell from patients with tuberculosis and to realize the role of memory T cells in the pathogenesis of tuberculosis. Methods Total CD4 memory T cells in the peripheral blood of patients with tuberculosis were detected by labeling with CD4,CCR7 and CD45RA fluorescence-antibodies and analyzing with flow cytomety. Mycobacterial antigen-specific memory T cells were detected by labeling with CD4, CD154, CCR7 and CD45RA fluorescence-antibodies . Understand the memory T cells distribution characteristics by comparison. Re-Sults In total CD4 + T cells from the patients with tuberculosis, the frequencies of CD45RA+ CCR7 + T cell ,CD45RA- CCR7 + T cell and CD45RA- CCR7+ T cell subsets were (59. 18 ± 16. 11) % , (15.40 ±6.48) % and (16.30 ±9.97)% separately. In the population of mycobacterial antigen-specific CD4 T cells, the frequencies of the three subsets were (32. 67 ± 13. 94)% , (56. 96 ± 12.70) % and (8.76 ±5. 62) % separately. Statistical analysis indicated that there were significant differences between the three subsets in the population of total CD4 T cells and that in mycobacterial antigen-specific CD4 T cells. Conclusion The total CD4 T cells contains mostly primitive T cells. However,in the antigen-specific CD4 T cells were population,the memory T cells are most frequent subset. This result indicates that antigen-specific memory T cells were produced in patients of tuberculosis and they might play an important role is protective immunity to prevent or decrease the severity of recurrent TB infection.
