胎儿心脏五步检查法诊断胎儿圆锥动脉干畸形的临床研究

摘要

Objective To study the validity of detecting the fetal conotruncal abnormalities of prenatal diagno-sis with five-step checking method. Methods Twenty thousand one hundred and thirty-three cases with fetal hearts were checked with five-step checking method in order to detect the heart abnormalities. Then analyze the cases with conotruncal abnormalities by prenatal diagnosis. Results Eighty-four cases with conotruncal abnormalities were con-firmed, among which there were 40 cases with TOF, 16 cases with TGA, 13 cases with DROV, 10 cases with PA/VSD and 5 cases with PTA. Among the 84 cases, 78 cases correspond with antenatal diagnosis and 6 cases not. Conclusion As a diagnosis of exclusion, most of the fetal conotruncal abnormalities can be diagnosed by using five-step checking method. It should be popularized in basic hospitals.%目的 探讨五步检查法在产前诊断胎儿圆锥动脉干畸形的有效性. 方法 应用五步检查法对20 133例孕妇进行胎儿心脏产前超声筛查,检出心脏畸形,对其中合并圆锥动脉干畸形者进行分析.结果 产前共诊断圆锥动脉干畸形84例,其中法洛四联症(TOF)40例、大动脉转位(TGA)16例、右室双出口(DROV)13例、肺动脉闭锁合并室间隔缺损(PA/VSD)10例、永存动脉干(PTA)5例. 84例中78例产前与产后诊断基本相符,6例产前与产后诊断不符. 结论 五步检查法可对绝大部分的胎儿圆锥动脉干畸形进行排除性诊断,易于在基层医院推广应用.
