弹性成像在乳腺影像学报告及数据系统4类肿块中的临床应用价值

         

摘要

目的:探讨弹性成像在乳腺影像学报告及数据系统(BI﹣RADS)4类肿块中的临床应用价值。方法选择常规超声诊断为BI﹣RADS 4类179个乳腺肿块进行弹性超声,以病理结果为金标准,比较分析单纯使用BI﹣RADS分类法、BI﹣RADS分类结合弹性成像后对BI﹣RADS 4类乳腺肿块诊断的敏感性、特异性、阳性预测值、阴性预测值、准确率。结果 BI﹣RADS分类法对乳腺肿物诊断的正确率71.5%,敏感性74.4%,特异性69.1%,阳性预测值67.0%,阴性预测值76.7%,结合弹性超声检查后,对乳腺肿物诊断的正确率90.5%,敏感性89.0%,特异性91.8%,阳性预测值90.1%,阴性预测值90.8%,组间差异比较差异有统计学意义(P<0.05)。结论对乳腺BI﹣RADS 4类肿块,联合超声弹性成像检查可提高乳腺肿瘤诊断的准确性。%Objective To investigate the clinical value of ultrasonic elastography (UE) in diagnosis of BI﹣RADS classification 4 breast lesions. Methods One hundred and seventy﹣nine breast lesions were examined by con﹣ventional ultrasonic and diagnosed as BI﹣RADS4,then UE were performed,the diagnostic value comparison between BI﹣RADS classification and UE combined with BI ﹣RADS classification was applied based on pathologic analysis. Results The sensitivity,specificity,positive predictive rate,negative predictive rate and accuracy by conventional ultrasonic were 74.4%,69.1%,67.0%,76.0%,71.5%, respectively. The sensitivity,specificity,positive predictive rate,negative predictive rate and accuracy rate of BI ﹣RADS classification which was combined with UE in diagnosing breast lesions were 89.0%,91.8%,90.1%,90.8% and 90.5%,respectively,which were statistically significant for each other (P<0.05). Conclusion UE combined with BI﹣RADS classification can improve the diagnostic accuracy for BI﹣RADS classification 4 breast lesions.
