甲状腺结节钙化在超声诊断甲状腺癌中的价值探讨

     

摘要

目的:分析甲状腺结节钙化灶在超声诊断甲状腺癌中的价值,为甲状腺癌的临床诊断提供参考。方法选取2015年1月至10月经我院收治的76例甲状腺结节同时伴有钙化的患者作为研究对象,将其术前钙化灶的超声诊断情况与经临床病理证实的结果作对比,探讨患者钙化灶与甲状腺癌之间的关系。结果76例甲状腺结节钙化患者中有45例为良性结节,包含微钙化13例以及粗钙化32例;31例为恶性结节,包含微钙化22例以及粗钙化9例,包含乳头状癌15例,滤泡性癌9例,髓样癌3例,鳞癌2例,转移癌1例以及未分化癌1例;恶性结节患者的钙化结节单发、边界不清晰以及钙化灶微钙化比例均明显高于良性结节患者,其差异具有统计学意义(P<0.05)。结论甲状腺结节钙化可以作为甲状腺癌超声诊断与鉴别的重要依据,临床应用价值较高。%Objective To analyze the value of thyroid nodule calcification in the ultrasonic diagnosis on thyroid carcinoma, and to provide reference for clinical diagnosis of thyroid carcinoma. Methods From January to October in 2015, 76 patients with thyroid nodules with calcification were selected as the research object. The preoperative calci-fications of the ultrasonic diagnosis case compared with the confirmed clinical pathology results, and the connection between calcifications in patients with thyroid cancer was explored. Results In the 76 patients with thyroid nodule calcification, 45 cases were benign nodules, including 13 cases containing microcalcification and 21 cases containing coarse calcification. Thirty-one cases were malignant nodules, including 22 cases containing microcalcification and 9 cases containing coarse calcification, and including 15 cases with papillary carcinoma, 9 cases with follicular carcino-ma, 3 cases with medullary carcinoma, 2 cases with squamous carcinoma, 1 case with metastatic carcinoma and 1 case with anaplastic carcinoma. In patients with malignant nodules, the ratio of patients with calcified nodules single-shot, unclear boundary and calcifications microcalcification were significantly higher than those of patients with benign nodules, the difference was statistically significant (P<0.05). Conclusion Thyroid nodule calcification can be used as an important basis for ultrasonic diagnosis on thyroid carcinoma and the clinical value is high.
