非酒精性脂肪性肝病患者血清脂联素和瘦素水平变化

摘要

目的 研究血清脂联素和瘦素水平与非酒精性脂肪性肝病(NAFLD)的关系.方法 应用放射免疫分析法检测56例NAFLD患者和42例健康人血清脂联素、瘦素、总胆固醇、甘油三酯、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、载脂蛋白ApoA、ApoB、胰岛素、胰岛素抵抗指数、体质量指数.结果 对照组和NAFLD患者血清脂联素分别为12.4±3.2mg/L和6.1±1.9mg/L (P＜0.01),高密度脂蛋白胆固醇为1.2±0.3mmol/L和1.0±0.3mmol/L(P＜0.05),瘦素为5.3±1.4μg/L和9.2±2.1μg/L(P＜0.01),胰岛素为9.4±4.2IU/L和18.5±7.8IU/L(P＜0.01),甘油三酯为1.0±0.5mmol/L和2.4±1.2mmol/L (P＜0.01),低密度脂蛋白胆固醇为2.2±0.6mmol/L和2.8±0.7mmol/L(P＜0.05),体质量指数为21.5±2.1 kg/m2和28.3±3.2kg/m2(P＜0.0),胰岛素抵抗为1.1±0.4IU/mmol和3.8±2.21u/mmol(P＜0.0l)；脂联素水平与瘦素、体质量指数和胰岛素抵抗呈负相关(r=-0.552,-0.497,-0.513,P均＜0.01).结论 NAFLD患者血清脂联素水平下降,瘦素水平升高,且与胰岛素抵抗相关.%Objective To study the relationship of serum adiponectin and leptin (Lp)levels in patients with non-alcoholic fatty liver disease(NAFLD).Methods Adiponectin,Lp,cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol (HDL-C),low density lipoprotein cholesterol (LDL-C),insulin,homeostasis model assessment index for insulin resistance(HOMA-IR),body mass index(BMI)were determined in 42 health persons and 56patients with NAFLD.Serum adiponectin and Lp levels were measured by radioimmunoassay.Results The serum adiponectin levels in NAFLD group was significantly lower than in control group(6.1±1.9mg/L vs.12.4±3.2,P＜0.01);The HDL-C levels was significantly lower in NAFLD group than in controls(1.0±0.3mmol/L vs.1.2±0.3 mmol/L,P＜0.05),whereas Lp was significantly higher in NAFLD group than in controls (9.2±2.1μg/L vs.5.3±1.4 μg/L,P＜0.01);In addition,the serum TC,LDL-C,insulin,and HOMA-IR and BMI in patients with NAFLD were higher than in control group (P＜0.01 or P＜0.05,respectively);Adiponectin was in negative relationship with Lp,HOMA-IR and BMI (r=-0.552,-0.497,-0.513,respectively,P＜0.01).Conclusion The adiponectin concentration is significant lower and the Lp concentration is significant high in patients with NAFLD.
