肝癌介入治疗患者心理韧性发展过程的质性研究

摘要

Objective To explore the process of resilience development in patients undergoing transarterial chemoembolization (TACE) for hepatocellular carcinoma (HCC).Methods Semi-structured interviews were conducted among 16 HCC patients treated with TACE.The interview data were analyzed using Colaizzi's phenomenological method and the Nvivo10 software.Results Three themes were identified:resilience crisis,adjustment and adaptation.Conclusion Patients use cognition,spirit,mental adjustment and behavioral control to cope with resilience crisis and then adapt.Lack of self-management knowledge was a barrier to resilience development,indicating education and support should be provided.%目的 探讨肝癌介入治疗过程中患者心理韧性的发生及发展过程.方法 采用质性研究中的现象学研究方法,对16例肝癌介入治疗患者进行半结构式访谈,根据Colaizzi 7步分析法以及Nvivo10辅助分析资料.结果 共提炼出经历心理韧性危机、加强韧性调控和适应状况3个主题.结论 肝癌介入治疗患者通过认知、精神、心态调节和行为控制来应对心理韧性危机,逐渐适应现状;缺乏自我管理知识是阻碍心理韧性发展的主要因素,需要医护人员加强健康教育与相关支持.
