儿科营养不良评估筛查工具用于住院患儿营养风险筛查的诊断效能研究

摘要

Objective To investigate the value of nutritional screening tool Screening Tool for the Assessment of Malnutrition in Pediatrics (STAMP) in screening malnutrition among hospitalized children.Methods A total of 808 hospitalized children were enrolled for malnutrition screening in a comprehensive children's hospital in Shanghai, from June to July 2016.Results A total of 137 cases were in low risk (16.96％), 604 cases in moderate risk (74.75％) and 67 cases in high risk (8.29％) according to the criteria of STAMP.Taking the cut-off point of 2 in STAMP, the sensitivity and specificity of the tool were 92.50％ and 21.70％, respectively.The positive predictive value and negative predictive value were 36.95％ and 85.40％, respectively.The area under the ROC curve was 0.622.Conclusion It is important to screen the nutritional status of hospitalized children.The cut-off point of 2 in STAMP shows a high sensitivity to find children with malnutrition risk.However, its specificity is relatively low.%目的 探讨儿科营养不良评估筛查工具在筛查住院患儿营养不良风险中的诊断价值.方法 对2016年6月至7月上海市某综合性儿童专科医院的808例住院患儿进行营养风险筛查.结果 用儿科营养不良评估筛查工具筛查出营养不良低风险患儿137例 (16.96％) 、中风险患儿604例 (74.75％) 、高风险患儿67例 (8.29％) .以总分2分作为临界点, 其灵敏度为92.50％, 特异度为21.70％, 阳性预测值为36.95％, 阴性预测值为85.40％;儿科营养不良评估筛查工具的ROC曲线下面积为0.622.结论 住院患儿是营养不良的高危人群, 以儿科营养不良评估筛查工具总分2分作为临界点时, 筛查的灵敏度较高, 能够筛查出营养不良风险的患儿, 但特异度较低.
