肺癌合并肺血栓栓塞抗凝治疗后血浆蛋白 C、抗凝血酶及凝血因子Ⅷ活性的变化

摘要

①目的探讨肺癌合并肺血栓栓塞抗凝治疗后蛋白 C、抗凝血酶及凝血因子Ⅷ活性变化。②方法我院接受治疗的肺癌合并肺血栓栓塞患者90例，随机分为观察组和对照组，各45例。对照组采用低分子肝素治疗，观察组在低分子肝素治疗的基础上加服华法林，采用序贯抗凝治疗。比较两组患者血浆蛋白 C、抗凝血酶及凝血因子Ⅷ活性变化。③结果治疗7天后，观察组血浆蛋白 C 含量（185．2±12．6），对照组（143．7±10．7），均高于治疗前，且观察组高于对照组，差异有统计学意义（P ＜0．05）；观察组抗凝血酶（110．8±8．9），对照组的（94．6±8．1），均高于治疗前，且观察组高于对照组，差异有统计学意义（P ＜0．05）；观察组凝血因子Ⅷ（253．1±18．7），对照组（198．3±17．9），均高于治疗前，且观察组高于对照组差异有统计学意义（P ＜0．05）。④结论血浆蛋白 C、抗凝血酶及凝血因子Ⅷ活性变化可以反映肺癌合并肺血栓栓塞抗凝治疗后的高凝状态，可作为肺癌合并肺血栓栓塞治疗的评价指标。%Objective To explore the activities changes of protein C,antithrombin and coagulation factor Ⅷ with the treatment of lung cancer combined with pulmonary embolism.Methods A total of 90 lung cancer patients with pulmonary embolism were randomly divided into treatment group and control group.the treatment group on the basis of low molecular heparin therapy with warfarin,and the control group treated with the low molecular heparin only.The protein C,antithrombin and coagulation factor Ⅷ were compared.Results After treatment 7 days,the level of protein C(185.2±12.6)of treat-ment group was higher than that of control group(143.7±10.7),and higher than before the treatment of two groups.There was statistically significant difference.The level of antithrombin(110.8±8.9)of treatment group was higher than that of control group(94.6±8.1),and higher than before the treat-ment of two groups.There was statistically significant difference.The level of coagulation factor Ⅷ(253.1±18.7)of treatment group was higher than that of control group (198.3±17.9),and higher than before the treatment of two groups.There was statistically significant difference.Conclusion The level of protein C,antithrombin and coagulation factor Ⅷ can reflect the treatment of lung cancer.They can be used as evaluation indexes of lung cancer with pulmonary embolism.
