Objective:To observe the effecacy and toxicity of low-dose non-platinum-based chemotherapy in treating elderly patients with non-small cell lung cancer.Methods:Fourty-three elderly patients with non-small cell lung cancer were randomly allocated into two groups:Patients in the study group received gemcitabine plus vinorelbine chemotherapy,and patients in the control group received gemcitabine plus cisplatin chemotherapy or vinorelbine plus cisplatin chemotherapy.Results:Of 23 cases in the study group,none received complete response,10 cases partial response,6 cases stable disease,7 cases progressive disease.The overall response rate was 43.5％.Of 20 cases in the control group,none received complete response,9 cases partial response,5 cases stable disease,6 cases progressive disease.The overall response rate was 45.0％.The overall response rates in the two groups were similar (P=0.9536).Up-grade Ⅱ toxicities of myelosuppression in the two groups were respectively 17.3％ and 21.3％ (P=0.5215).Up-grade Ⅱ toxicities of gastrointestinal in the two groups were respectively 15.3％ and 70.1％ (P ＜0.0001).The improvement rates of Karnofsky performance status score in the two groups were respectively 60.9％and 30％ (P =0.0429).Conclusion:Low-dose non-platinum-based chemotherapy should be considered safe and effective chemotherapy regimen for elderly patients with non-small cell lung cancer.%目的:探讨低剂量非铂类联合化疗方案在老年晚期非小细胞肺癌患者中的疗效及安全性.方法:将43例晚期非小细胞肺癌患者随机分成观察组和对照组,分别采用吉西他滨加去甲长春花碱方案和吉西他滨加顺铂或去甲长春花碱加顺铂方案进行治疗.结果:观察组23例中完全缓解0例,部分缓解10例,稳定6例,进展7例,总有效率43.5％,对照组20例中完全缓解0例,部分缓解9例,稳定5例,进展6例,总有效率45.0％,两组总有效率无统计学差异(P =0.9536).两组Ⅱ度及以上骨髓抑制发生率分别为17.3％和21.3％,无统计学差异(P=0.5215),Ⅱ度及以上消化道反应发生率分别为15.3％和70.1％,差异有统计学意义(P ＜0.0001),KPS评分增高率分别为60.9％和30％,差异有统计学意义(P =0.0429).结论:低剂量非铂类联合化疗治疗老年晚期非小细胞肺癌安全性较高,疗效肯定,可以在临床中应用.
