超声造影评价原发性肝癌DSA引导下肝动脉化疗栓塞治疗的疗效

         

摘要

目的:研究超声造影对原发性肝癌肝动脉化疗栓塞治疗的疗效评价.方法:选取2013年1月至2014年1月我院接诊的80例原发性肝癌患者作为本次研究对象.DSA组采用DSA引导下肝动脉化疗栓塞进行治疗,对照组采用手术治疗,观察DSA引导下肝动脉化疗栓赛治疗后单发与多发病灶超声造影结果、原发性肝癌患者治疗后1个月AFP值的变化情况、DSA组与对照组的生存率比较.结果:治疗后经过检查,DSA组单发病灶25例,多发病灶15例;对未完全灭活的肿瘤再行DSA下肝动脉化疗栓塞治疗(直至完全灭活),DSA组AFP值下降程度优于对照组[82.5% (33/40)vs 60.0%(24/40)];6个月、1年、2年DSA组生存率均优于对照组[100.0% (40/40) vs 85.0%(34/40)、90.0% (36/40) vs 72.5% (29/40)、80.0% (32/40) vs57.5% (23/40)].结论:在DSA引导下肝动脉化疗栓塞对原发性肝癌进行治疗时,超声造影可以对疗效进行评价,并且起着补充治疗的作用,利于病灶得到完全灭活,使生存期得到延长,值得应用推广.%Objective:To investigate the value of contrast-enhanced ultrasound in evaluating the therapeutic effect of transcatheter arterial chemoembolization for primary hepatocellular carcinoma.Methods:The data of 80 patients with primary liver cancer from January 2013 to January 2014 in our hospital were researched.DSA group was treated with DSA in the treatment of hepatic artery chemoembolization,control group was treated with operation.Observation of single and multiple lesions after DSA ultrasound imaging results, 1 month after the treatment of patients with primary liver cancer AFP changes in the situation, DSA treatment group compared with the survival rate of the control group.Results:After treatment and examination,25 cases with single lesion, 15 cases with multiple lesion, for the patients who did not completely eliminate the DSA in the treatment of hepatic artery chemoembolization,the AFP value of the treated patients was better than that of the control group[82.5% (33/40) vs 60.0% (24/40)].6 months, 1 year,2 years of DSA group were better than the control group[100.0% (40/40) vs 85.0% (34/40),90.0% (36/40) vs 72.5% (29/40),80.0% (32/40) vs 57.5% (23/40)].Conclusion:In the use of chemoembolization of hepatic artery under DSA on primary liver cancer treatment and contrast enhanced ultrasound can and the curative effect was evaluated.It plays a supplementary treatment role, conducive to the lesions were completely inactivated, survival period was extended, it is worthy of popularization and application.
