尿液HCMVDNA检测在儿童HCMV感染中的临床应用

         

摘要

目的:对患儿尿液进行 HCMV DNA荧光定量检测并对检测结果进行统计学分析,以了解 HCMV在患儿各年龄段及各种疾病中的感染情况。方法采用荧光定量PCR技术对2014年1月~2015年12月收集的1492份尿液标本进行 HCMV DNA荧光定量分析。结果新生儿组、婴幼儿组、学龄前组和学龄期组之间的 HCMV DNA检测结果比较差异有统计学意义(χ2=9.253~23.452,P 均<0.01);学龄期组与新生儿组比较差异有非常显著性统计学意义(χ2=23.452,P<0.001)。患儿常见几种疾病组中,肝功能异常组、支气管肺炎组和发热组之间差异无统计学意义(χ2=0.356~0.865,P均>0.05)。肝功能异常组、支气管肺炎组和发热组三组与早产儿组和高胆红素血症组比较差异有统计学意义(χ2=13.231,P均<0.01)。结论新生儿和婴幼儿是人巨细胞病毒感染的高发人群,其中的早产儿和高胆红素血症患儿的 HCMV DNA检出率较高应引起重视;尿液 HCMV DNA检测简便快速,标本留取方便,在婴幼儿 CMV感染的诊断和治疗中具有重要意义。%Objective To study the infection state of HCMVin different age of children and co-infection with other diseases, detecting human cytomegalovirus (HCMV)in urine of 1 492 cases of children by fluorescence quantitative detection meth-od.Then the detection results were retrospectively analyzed.Methods Collected 1 492 specimens of children’s urine from January 2014 to December 2014.Detected HCMV by fluorescence quantitative method.Results For different ages with HC-MV DNA detection results,there were significant statistical differences between school-age group,neonatal group,infant group and preschool group respectively (χ2=9.253~23.452,P<0.01).Especially there was very significant statistical differences between neonatal group and school-age group (χ2=23.452,P<0.001).For different co-infection diseases with HCMV DNA detection results,there was not significant statistical difference between fever group and abnormal liver func-tion group,bronchial pneumonia group respectively (χ2=0.356~0.865,P>0.05).Compared abnormal live function,bron-chial pneumonia group and fever three group with preterm group and high blood bilirubin group,the difference was statisti-cally significant (χ2=13.231,P均<0.01).Conclusion The results showed that high infection rate of HCMV in newborn infants and infants.Should pay attention to high blood bilirubin and premature infant co-infection with HCMV.Urine speci-men is rapid and convenient specimen for detection HCMV of infants.It is great significance in diagnosis and treatment of in-fantile CMV infection.
