经尿道膀胱肿瘤电切术治疗高龄膀胱癌合并反复性出血的应用体会

摘要

Objective:The aim of this study was to evaluate the efficacy and security of transurethral resection of bladder tumor (TURBT)for bladder carcinoma with recurrent bleeding in elderly patients.Methods:62-patient were selected and treated with TURBT between May 2003 and May 2012,and all people were diagnosed as bladder transi-tional cell carcinoma by pathology after operation.Results:All people were operated a successful TURBT,without intraoperative blood transfusion or intraoperative death.Conventional postoperatively with mitomycin 40 mg was di-luted by 40 mL saline water for bladder perfusion chemotherapy.All people postoperatively information were collect-ed by telephone,and the follow-up time from 6 to 60 months,the average time was 32 months.62-patient achieved well hemostasis and significantly improved the quality of the life.Moreover,the 5-year survival rate was arrived 79.0% after TURBT.Conclusions:The effect of TURBT treatment of the bladder carcinoma patients with recurrent bleeding reached at 100%,and improved the quality of the life for therapeutic purposes.%目的：：评价经尿道膀胱肿瘤电切术(transurethral resection of bladder tumor,TURBT)在治疗高龄膀胱癌合并反复性出血中的疗效及安全性.方法：2003年5月~2013年5月共收治高龄膀胱癌合并反复性出血患者62例,行TURBT治疗.结果：62例患者手术均顺利完成,无术中死亡,无术中输血；术后常规用丝裂霉素40 mg＋0.9％生理盐水40 ml 进行膀胱灌注化疗.术后采用电话随访,随访时间为6~60个月,平均32个月；均能达到良好止血效果,患者生活质量明显改善,5年生存率为79.0％.结论：TURBT处理高龄膀胱癌合并反复性出血止血效果达100％,安全系数高,术后配合丝裂霉素化疗,达到治疗及改善生存质量的目的.
