不同浓度A型肉毒毒素治疗偏侧面肌痉挛的临床疗效

摘要

Objective:To investigate the clinical effects and security of botulinum toxin A(BTX-A)at different dilu-tions injection in the treatment of hemifacial spasm.Methods:A total of 98 patients with hemifacial spasm were selected for the study.They were randomly assigned into two groups.There were 49 patients with hemifacial spasm in each group.The high-density group was given to injection of high-density(50U/ml),the low-density group was given to in-jection of low-density(25U/ml).They were subjected to repetitive injection of botulinum toxin A of same concentra-tions after recurrence.We observed for 1 year.The clinical effects and security of botulinum toxin A were compared between two groups.Results:Compared with the low-density group,the overall efficiency of was significantly increased、the onset time was significantly shorten、the lasting time was significantly prolonged and the Cohen spasms strength grade scores was significantly decreased in the high-density group(all P <0.05 ).But compared with the low-density group,the drug adverse reaction in the high-density group was not increased(P >0.05).Conclusion:This study showed that injection of botulinum toxin A with different concentrations in patients with hemifacial spasm was safe and effec-tive.With high-density,the overall efficiency was increased and the drug adverse reaction was not increased.%目的：：探讨不同浓度 A 型肉毒毒素(BTX-A)治疗偏侧面肌痉挛(HFS)的临床疗效及安全性。方法：选取2010年10月-2014年6月入住我院神经内科明确诊断为 HFS 的患者98例,分为低浓度组和高浓度组,每组49例,高浓度组予多点局部肌肉注射50U/ml BTX-A,低浓度组予多点局部肌肉注射25U/ml BTX-A,复发后予重复给药,临床随访观察1年,分析两组药物的临床疗效及药物不良反应的发生率。结果：与低浓度组比较,高浓度组治疗 HFS 的总体有效率明显升高,药物起效时间明显缩短,药物持续时间明显延长,Cohen 分级明显降低,差异有统计学意义(均 P<0.05)。而与低浓度组比较,高浓度组药物不良反应的发生率无明显增加,差异无统计学意义(P >0.05)。结论：BTX-A 多点局部肌肉注射治疗 HFS 安全有效,高浓度(50U/ml)的总体有效率明显提高,且药物不良反应无明显增加。
