持续非卧床腹膜透析患者血清25（OH）D3水平及其与心功能的相关性分析

摘要

Objective :To investigate the levels of serum 25 Hydroxy Vitamin D3 in patients with continuous ambulato‐ry peritoneal dialysis and its correlation with cardiac function .Methods :100 patients with continuous ambulatory perito‐neal dialysis (CAPD group) ,100 patients with healthy subjects (control group) were collected at the Renal Internal Medicine department of our hospital were chosen for the study .The 25(OH)D3 ,BNP levels were detected with En‐zyme linked immunosorbent assay .The parameters of left atrial diameter (LAD) ,left ventricle end‐diastolic diameter (LVEDD) ,left ventricle ejection fraction (LVEF) ,fraction shortening (FS) were detected with echocardiography .Re‐sults:Compared with the control group ,the levels of BNP ,LAD and LVEDD in CAPD group were significantly in‐creased .The 25(OH)D3 ,LVEF ,FS were significantly decreased .Correlation analysis revealed that the levels of 25 (OH)D3 was positively correlated with LVEF and FS (r=0 .745 ,0 .457 ,both P<0.05) ,and were negatively correlated with BNP ,LAD and LVEDD (r= -0 .632 ,-0 .701 ,-0 .564 ,all P<0 .05) .Conclusion:The levels of serum 25 Hy‐droxy Vitamin D3 in patients with continuous ambulatory peritoneal dialysis was significantly decreased ,which was closely associated with cardiac function .%目的：探讨持续非卧床腹膜透析（C A PD ）患者血清25（O H ）D3水平及其与心功能的相关性分析。方法：选择2013年1月－2015年10月在我院肾内科门诊随访，行C A PD治疗时间超过6个月的患者100例（C A PD组），同时选取同时期门诊健康体检者100例（正常对照组）为研究对象，采用酶联免疫吸附试验检测两组实验对象25（O H ）D3、BNP浓度，采用超声心动图检测左房舒张末期内径（LAD）、左室舒张末期内径（LVEDD）、左室射血分数（LVEF）、短轴缩短率（FS）。结果：与正常对照组比较，CAPD组患者25（OH）D3、LVEF、FS 均明显降低，BNP、LAD、LVEDD、SBP、DBP均明显升高，差异有统计学意义（P均＜0．05）。单因素分析显示：CAPD患者血清25（OH）D3浓度与BNP、LAD、LVEDD呈明显负相关（r分别为－0．632、－0．701、－0．564，P均＜0．05），与LVEF、FS呈明显正相关（r分别为0．745、0．457，P均＜0．05）。结论：C A PD患者血清25（O H ）3浓度明显下降，其与心功能相关指标有良好的相关性，25（OH）D3可作为判断CAPD患者心功能的良好指标。
