爱惜邦线U形缝合联合聚髌器治疗髌骨下极骨折

摘要

Objective:To investigate the clinical efficacy of surgical treatment of inferior polar fracture of patella by U-shaped Ethibond combined with patellar concentrator.Methods:Between April 2009 and September 2015,68 patients were diagnosed with comminuted inferior polar fracture of patella.35 cases of inferior polar fracture of the patella (group A) were operated by U-shaped Ethibond combined with patellar concentrator,and 33 cases (group B) were operated by steel wire cerclage combined with patellar concentrator.The range of motion(ROM) and the Lysholm knee scores were applied to evaluate function of the knee.Results:30 patients of group A and 28 patients of group B were followed up for 8~10 months,the mean follow-up period was 8.35±0.68 months.At the last follow-up.The range of motion (ROM) between two groups [A group:(133.32±7.15)°,B group:(121.78±4.56)°] had statistical significance (P<0.05).According to the Lysholm scores,in group A the excellent-good rate was 96.67%,and the scores were 96.48±5.79;in group B,the excellent-good rate was 89.29%,and the scores were 91.73±4.76,and the scores between two groups had statistical significance (P<0.05).Conclusion:U-shaped Ethibond combined with patellar concentrator is an effective method to treat the comminuted inferior polar fracture of patella with stable fixation,early start functional training of knee joint,favorable recovery and deserving the clinical promotion.%目的:探讨爱惜邦线U形缝合联合聚髌器治疗髌骨下极骨折的临床疗效.方法:2009年4月-2015年9月收治68例髌骨下极粉碎性骨折患者,其中35例(A组)采用爱惜邦线U形缝合联合聚髌器术式,33例(B组)采用钢丝环扎联合聚髌器术式.术后膝关节功能情况予膝关节活动度 (Range of motion,ROM)及Lysholm评分表进行评价.结果:A组及B组病例中获得随访的分别为30例及28例,术后平均随访(8.35±0.68)个月.终末随访行患侧膝关节活动度(ROM)测量:A组(133.32±7.15)°,B组(121.78±4.56)°,组间差异有统计学意义(P<0.05).采用Lysholm评分:A组平均为 (96.48±5.79)分,优良率达到96.67%;B组为 (91.73±4.76)分,优良率达89.29%,组间差异有统计学意义(P<0.05).结论:爱惜邦线U形缝合联合聚髌器治疗髌骨下极骨折,固定骨折牢靠,术后可提早功能锻炼,膝关节功能恢复满意,值得临床推广.
