MR氢质子波谱成像在定量诊断脂肪肝中的临床应用

         

摘要

目的 探讨1H磁共振波谱(1 H-MRS)定量分析方法对脂肪肝诊断的可行性.方法 选取48例临床明确诊断的非酒精性脂肪肝患者及20例健康志愿者行1 H-MRS扫描和CT平扫,1 H-MRS测量脂质峰值和水峰峰值,计算脂肪分数,并与CT肝/脾比值所得结果进行比较.结果 正常组和脂肪肝组水峰峰值无明显差异,而脂质峰值有显著差异.2例CT肝/脾比值诊断脂肪肝患者在波谱成像时未出现明显峰值,其余1H-MRS所得波谱曲线与CT肝/脾比值诊断结果一致.结论 MR氢质子波谱成像技术可以定量诊断脂肪肝.%Objective To evaluate the feasibility of quantitative diagnosing fatty liver by 1H-MR spectroscopy (1H-MRS). Methods 48 cases of nonalcoholic fatty liver verified clinically and 20 healthy volunteers underwent 1H-MRS scan plain CT scan. 1H-MRS sequence measured the peak of h2o and lipid. The relative contents of the lipid compound were calculated, and compared with the results of CT value ratio (liver and spleen). Results On 1H-MRS, the value of h2o peak had no obvious difference between the normal group and fatty liver group, but, the value of lipid peak had obvious difference between the two groups. The results were in accordance with the results of CT value ratio (liver and spleen) except in two cases. Conclusion 1H-MRS imaging provides the feasibility of quantitative detecting fatty liver.
