坎地沙坦联合氢氯噻嗪治疗老年高血压患者的疗效及对靶器官的保护作用

摘要

Objective:To observe the effect of the candesartan in combination with hydrochlorothiazide in elderly patients with high blood pressure,as well as the protection of renal function and the clinical effect of reversing left heart hypertrophy.Methods:1 1 2 elderly patients with high blood pressure were randomly di-vided into observation group(56 cases)and control group(56 cases)from January 2010 to June 2010 in our hospital .Two groups of patients were given candesartan 8 mg/d treatment,observation group based on this, top allied with hydrochlorothiazide 12.5mg/d.To observe the treatment effect of two groups in blood pres-sure levels,as well as left ventricular mass index(LVM),left ventricular mass index(LVMI)andthe renal function of various indicators after 6 months.Results:After completion of therapy,the total step-down effec-tive rate was 89.3% in the observation group and 71.4% in the control group.Compared the total effective rate of two groups,the difference is statistically significant(P<0.05);LVM and LVMI value observed after observation group was obviously lower than the control group,the difference is statistically significant(P<0.05);the renal function indicators of two groups has no statistical significance(P>0.05).Conclusion:The treatment effect of senile hypertension antihypertensive with candesartan in combination with hydrochlorothi-azide is superior to the single drug,and the combination in reverse left ventricular hypertrophy curative effect is more significant,but it has no obvious advantage in the combination therapy of renal protection.%目的：：观察坎地沙坦联合氢氯噻嗪治疗老年高血压患者的疗效，以及其保护肾功能、逆转左心肥厚的临床效果。方法：选取2010年1月~2012年6月在某院进行治疗的112名老年高血压患者，将其随机分为观察组(56例)和对照组(56例)。两组患者均给予坎地沙坦8mg/d治疗，观察组在此基础上联用氢氯噻嗪12.5mg/d。治疗6个月后观察两组患者血压水平，以及左心室肌质量(LVM)、左心室重量指数(LVMI)和肾功各项指标。结果：治疗6个月后，观察组降压总有效率为89.3%，对照组为71.4%，两组相比总有效率差异具有统计学意义(P<0.05)；治疗后观察组 LVM及 LVMI 值明显低于对照组，差异具有统计学意义(P<0.05)；治疗后两组患者肾功能各项指标比较差异无统计学意义(P>0.05)。结论：坎地沙坦联合氢氯噻嗪治疗老年高血压降压效果优于单药治疗，且联合用药在逆转左心室肥厚疗效更为显著，但在肾功能保护方面联合用药无明显优势。
