DWI和ADC值在脑脓肿包膜期与脑胶质瘤鉴别诊断中的应用

         

摘要

目的 探讨磁共振扩散加权成像(DWI)信号表现及表观扩散系数(ADC)值在脑脓肿包膜期与胶质瘤鉴别诊断中的应用.方法 通过病例回顾脑脓肿包膜期及胶质瘤的DWI的信号表现和ADC图的影像差异,并测量脑脓肿内及胶质瘤内的平均ADC值,和对照(MR对侧)ADC值,进行统计学分析.结果 (1)脑脓肿DWI为高信号,ADC图为低信号,而胶质瘤DWI表现为低信号,ADC图为高信号;(2)ADC值脑脓肿与对照组通过配对检验(P<0.01),具有统计学差异性.胶质瘤与对照组通过配对检验(P<0.01),具有统计学差异性;脑脓肿与胶质瘤通过独立样本检验(P<0.01),具有显著性差异.结论 DWI、ADC值对脑脓肿与胶质瘤的鉴别诊断具有一定意义.%Objective To study the clinical application of magnetic resonance diffusion weighted imaging (DWI) and apparent diffusion coefficient (ADC) value in the differential diagnosis between brain abscess (BA) in capsule-period and brain glioma.Methods By reviewing cases,DWI signal performance and the differences of ADC value of BA and brain glioma were found,and the average ADC values in brain abscesses cavity and glioma were measured,compared and statistically analyzed.Results (1) In BA cavity,DWI was characterized by high signal and the ADC showed low signal,while in glioma area,DWI was characterized by low signal and the ADC showed high signal.(2)As for the ADC value,there was significant difference in paired-sample tests between BA group and control group (P<0.01),and significant difference occurred between glioma group and control group (P<0.01),as well as in independent sample test of both BA group and glioma group (P<0.01).Conclusion DWI and ADC values can improve differential diagnosis between BA and brain glioma.
