65岁以上老年患者腹腔镜结直肠癌手术的短期疗效观察

摘要

Objective:To evaluate the short-term results of laparoscopic and open colorectal resection in elderly patients aged over 65years .Methods:From Jan.2012 to Sep.2014, laparoscopic colorectal resection was performed for 67 cases ( laparoscopic group),and 95 patients were treated with open surgery (open group).Clinical data were analyzed retrospectively ,including preopera-tive,intraoperative and postoperative indexes .Results:Between laparoscopic group and open group ,no significant difference was found in preoperative clinical data and the number of retrieved lymph nodes (P>0.05).However,the incision length,intraoperative blood loss ,postoperative ambulation time ,recovery of gastrointestinal function ,total postoperative complications ,and postoperative hospital stay were less in laparoscopic group than those in open group (P<0.05).Conclusions:Compared with traditional open operation ,laparo-scopic colorectal cancer operation for patients aged over 65 years is safe,effective and feasible ,has the same radical effects and many merits in short-term curative effect ,such as little interference and quick postoperative recovery .Laparoscopic operation can be recom-mended as routine operation for aged patients with colorectal cancer .%目的：比较为65岁以上老年患者行腹腔镜与开腹结直肠癌切除术的短期疗效。方法：将2012年1月至2014年9月收治的162例结直肠癌患者分为腹腔镜组（ n＝67）与开腹组（ n＝95），比较术前临床资料、术中及术后指标。结果：腹腔镜组与开腹组术前临床资料、清扫淋巴结数量差异无统计学意义（ P＞0．05）；腹腔镜组切口长度、术中出血量、首次下床活动时间、肛门排气时间、并发症发生率、术后住院时间优于开腹组（P＜0．05）。结论：为65岁以上的老年患者行腹腔镜结直肠癌切除术是有效、安全、可行的，与开腹手术相比具有相同的根治效果，对机体干扰小，术后康复快，短期疗效方面具有许多优点。腹腔镜手术可作为老年患者结直肠癌根治术的常规术式。
