Yes相关蛋白和β-链蛋白在乳腺浸润性导管癌中的表达及临床意义

     

摘要

目的 检测乳腺浸润性导管癌(非特殊型)、乳腺腺瘤、正常乳腺组织中Yes相关蛋白(YAP)和β-链蛋白的表达,分析YAP、β-链蛋白二者与乳腺浸润性导管癌临床病理因素之间的关系、表达的相关性、与乳腺浸润性导管癌预后的关系.方法 采用链霉卵白素-过氧化物酶法检测YAP和β-链蛋白在32例正常乳腺组织、40例乳腺腺瘤组织、97例乳腺浸润性导管癌组织中的表达.结果 YAP在正常乳腺组织、乳腺腺瘤组织、乳腺浸润性导管癌组织中的阳性表达率依次减低,差异有统计学意义(P< 0.05);β-链蛋白在正常乳腺组织、乳腺腺瘤组织、乳腺浸润性导管癌组织中的阳性表达率依次上调,差异有统计学意义(P<0.05).YAP表达与乳腺浸润性导管癌患者年龄、组织学分级、肿瘤大小、淋巴结转移、是否绝经等均无相关性(P> 0.05),与是否为三阴性乳腺癌相关(P< 0.05);β-链蛋白表达与患者年龄、肿瘤大小、是否绝经等均无相关性(P>0.05),与组织学分级、淋巴结转移、是否为三阴性乳腺癌相关(P< 0.05).YAP的阳性表达与乳腺浸润性导管癌患者的总生存率无相关(P> 0.05),与无复发生存率相关(P< 0.05);β-链蛋白阳性表达与乳腺浸润性导管癌患者的总生存率、无复发生存率均无相关(P>0.05);YAP(-)/β链蛋白(+)共表达与乳腺浸润性导管癌患者的总生存率、无复发生存率均相关(P<0.05).在乳腺浸润性导管癌、乳腺腺瘤组织中YAP和β-链蛋白的表达均呈负相关(P< 0.01);YAP和β-链蛋白二者在正常乳腺中的表达无相关性(P>0.05).结论 β-链蛋白在乳腺浸润性导管癌组织中呈高表达,并与淋巴结转移相关,可作为乳腺浸润性导管癌组织学分级判断的参考指标.在乳腺浸润性导管癌组织中YAP和β-链蛋白的表达呈负相关,且YAP(-)/β-链蛋白(+)共表达与乳腺浸润性导管癌患者的总生存率、无复发生存率均相关,提示二者联合检测有助于乳腺浸润性导管癌的临床诊断、疗效评估及预后判断.%Objective The expression of Yes associated protein (YAP) and β-catenin in human normal breast tissue,breast adenoma and breast infiltrating ductal carcinoma (if not otherwise specified) were studied,the relationship between the clinicopathological characters of breast infiltrating ductal carcinoma and two factors were analyzed,to further explore their roles in breast infiltrating ductal carcinoma occurrence,development process and prognosis.Methods The expression of YAP and β-catenin in 32 cases of human normal breast tissue,40 cases of breast adenoma and 97 cases of breast infiltrating ductal carcinoma was evaluated by immunohistochemical staining (streptavidine-peroxidase method).Results The expression of YAP in human normal breast tissue,breast adenoma and breast infiltrating ductal carcinoma decreased gradually,which in normal breast tissue was higher than in breast adenoma and breast infiltrating ductal carcinoma.A statistically significant result (P < 0.05) was produced.The positive expression of β-catenin was remarkably higher in breast adenoma and breast infiltrating ductal carcinoma than in normal breast tissue In human normal breast tissue,breast adenoma and breast infiltrating ductal carcinoma,the positive expression rate increased in turn,providinga statistically significant difference (P < 0.05).The expression of YAP was not statistically correlated with the age,histological grade,tumor size,lymph node metastasis,menopausal status of patients,etc.,yet it was associated with the triple-negative breast cancer (P < 0.05).There was no correlation between the expression of β-catenin and breast infiltrating ductal carcinoma patients' age,tumor size,menopausal status,etc (P >0.05),but the expression of β-catenin was statistically correlated with the histological grade,lymph node metastasis and triple-negative breast cancer (P < 0.05).The positive expression of YAP and infiltrating ductal carcinoma of the breast of overall survival rate in patients has no correlation (P > 0.05),which was associated with the relapse-free survival rate (P < 0.05).The positive expression of β-catenin was not statistically correlated with the overall survival rate and relapse-free survival rate (P > 0.05).Yet YAP(-)/β-catenin(+)was associated with overall survival rate and the relapse-free survival rate (P < 0.05).The expression of YAP and β-catenin were all negatively related in breast adenoma and breast infiltrating ductal carcinoma (rs negative,P < 0.01).These two proteins were not relevant in human normal breast tissue (P > 0.05).Conclusion The expression level of β-catenin in breast infiltrating ductal carcinoma was frequently up-regulated,and the positive expression of β-catenin was statistically correlated withlymph node metastasis,suggesting that β-catenin plays an important role in accelerating the occurrence and metastasis of breast infiltrating ductal carcinoma.The positive expression of β-catenin was statistically correlated with the histological grade in breast infiltrating ductal carcinoma,indicating that β-catenin can be used as a reference designator to estimate the tissue classification of breast infiltrating ductal carcinoma.The expression of YAP and β-catenin was negatively correlated in breast infiltrating ductal carcinoma (non specific);YAP(-)/β-catenin(+) was associated with the overall survival rate and relapse-free survival rate (P < 0.05).A combined detection of YAP with β-catenin will help clinically diagnose breast infiltrating ductal carcinoma and prognosis.
