超声引导下经皮穿刺热消融治疗甲状腺结节伴囊内出血的临床研究

摘要

目的：：研究超声引导下经皮穿刺热消融治疗甲状腺结节伴囊内出血的临床价值。方法：对34枚甲状腺结节(伴囊内出血)，在超声引导下经皮穿刺活检后行微波(射频)热消融治疗，评价消融区的坏死和体积缩小程度，以及消融术相关的副作用及并发症。结果：34枚伴囊内出血的甲状腺结节消融后经超声造影检查显示全部丧失血供，且消融区范围完全覆盖原结节大小；超声随访检查发现消融区在不同复查时间内体积缩小率不同，差异有统计学意义。结论：超声引导下伴囊内出血的甲状腺结节经皮热消融治疗安全性高，创伤小，可控性强，效果明显，具有临床应用潜质。%Objective:Study of ultrasound guided percutaneous puncture thermal ablation of thyroid nodules with intracapsular hemorrhage clinical value. Methods: To 34 thyroid nodules ( with intracapsular hemorrhage ) , under ultrasound guided percutaneous biopsy after microwave thermal ablation(Radio Frequency),evaluate the necrosis and shrinkage degree of ablation area,as well as ablation related side effects and complications. Results:After ablation of 34 thyroid nodules with intracapsular hemorrhage by contrast-enhanced ultrasound examination showed all the loss of blood supply,and the ablation area range is completely covered the original nodule size;Ultrasonic follow-up examination found that the ablation area in different shrinkage rate review time,Statistically significant differences. Conclusion: Ultrasound guided with intracapsular hemorrhage of thyroid nodules percutaneous thermal ablation of high security,small trauma,strong controllability,obvious effect,has potential clinical application.
