3D-CPA技术对正常妊娠不同孕周胎盘血流灌注的评价

     

摘要

目的:探讨应用三维彩色能量多普勒超声成像(three-dimensional color power angiographic,3D-CPA)技术定量评价正常妊娠不同孕周胎盘血流情况的价值.方法:选取100例正常孕妇,分为A组(孕周24+1~28周),B组(孕周28+1~32周),C组(孕周32+1~38周),对所有孕妇胎盘床血管进行三维重建,采用VOCAL软件测量VI、FI及 VFI并观察其变化,并与孕龄(gestational age,GA)进行比较,计算它们之间的相关关系.结果:B组及C组较A组可显示较多的绒毛干血管.各组的VI、FI、VFI值分别为:A组4.01±1.50、31.5±2.88、1.67±0.62;B组5.94±0.91、34.7±3.21、2.09±0.77;C组6.23±2.11、36.9±4.78、2.35±0.90.3组VI、FI、VFI值的比较:B组及C组较A组均升高且差别有统计学意义(P<0.05).B组较C组间差别均无统计学意义(P>0.05).结论:3D-CPA技术使定量评估胎盘血流灌注成为可能,是观察孕妇的胎盘血流灌注变化的有效工具.%Objective:To investigate the value of three-dimensional color power angiographic (3D-CPA) on assessment of placental function in normal pregnancy with different gestational age. Methods: A total of 100 normal pregnant women were divided into group A (gestational age between 24 1~28 weeks) , group B (gestational age bet-ween 28 1~32 -weeks) , and group C (gestational age bet-ween 32 1~38 -weeks). VI, FI and VFI -were detected by 3D-CPA in each group, and the relationship bet-ween VI, FI and VFI and the gestational age was determined. Results: More stem villous vessels were presented in groups B and C than in group A. VI, FI and VFI were 4. 01±1. 50, 31. 5 ± 2. 88 and 1. 67 ± 0. 62 in group A; 5. 94 ± 0. 91 , 34. 7 ±3. 21 and 2. 09 ± 0. 77 in group B; and 6. 23 ± 2. 11 , 36. 9 ± 4. 78 and 2. 35 ± 0. 90 in group C; respectively. The three-dimensional Doppler indices in groups B and C were significantly higher than those in group A ( P <0. 05). There were no significant differences in the three-dimensional Doppler indices between groups B and C ( P >0. 05). Conclusion: Three-dimensional color power angiographic make the quantitative evaluation of placental function to be possible, and it is an appropriate tool for routine evaluation of placental function.
