The mechanism of bone marrow relapse in pediatric acute lymphoblastic leukemia has been deeply researched in recent yfears. The roles of hematopoietic stem cells, some gene abnormalbilities including IKZF1, JAK, CRLF2 and CREBBP, as well as mutation of genes influencing durg resistence have been confirmed to related with relapse of disease. The microRNAs, gene expression profile of patient, microenvironment of bone marrow including bone marrow fibrosis, mesenchymal stem cells and matrix metalloprotease, also play an important role in the process of leukemia recurrence. This article reviewed the details mentioned above.%近年来,关于儿童急性淋巴细胞白血病复发机制方面的研究逐渐深入,其中涉及白血病干细胞、与白血病相关的多种基因异常,如IKZF1、JAK、CRLF2及CREBBP等,以及对糖皮质激素耐药有影响的多种基因突变都已经被证实与复发明显相关,而参与基因表达调控的microRNA、宿主基因型和骨髓微环境,如骨髓纤维化密度、间充质干细胞、基质金属蛋白酶等同样对复发有重要影响.本文重点综述以上几方面对儿童急性淋巴细胞白血病骨髓复发的影响.
