中药通腑开窍液灌肠在高血压性脑出血治疗中的应用∗

摘要

目的：观察中药通腑开窍液灌肠干预高血压性脑出血患者的临床效果并探讨其作用机理。方法将符合纳入标准的150例患者随机分为观察A组、观察B组、对照组共3组，每组患者均50例，对照组采用基础治疗加微创清除术；观察A组给予基础治疗加微创清除术加局部灌注醒脑静针剂（微创术中）；观察B组给予基础治疗加微创清除术加局部灌注醒脑静针剂（微创术中）加通腑开窍液灌肠。结果观察B组治疗后总有效率达86.00%，显著优于观察A组的74.00%及对照组的56.00%（P<0.05）。治疗后观察A组、B组促进颅内血肿吸收的效果较对照组为优（P<0.05）。治疗后观察A组和观察B组减轻脑水肿的效果均较对照组明显，尤以观察B组效果尤佳（P<0.05）；治疗后在改善神经功能方面，观察A组、B组较对照组为优（P<0.05），其中观察B组神经功能评分后所得积分值显著低于观察A组（P<0.05）。结论采用中药通腑开窍液灌肠干预高血压性脑出血，临床中取得了较好的疗效。%Objective:To observe the clinical effect of treating hypertension brain hemorrhage by stilling Tongfukaiqiao liquor into rectum and its mechanism. Methods:150 patients were divided into 3 groups stochas⁃tically:control group、treatment group A and treatment group B. On the basis of basic treatment,control group were given the treatment of micro creates encephalic haematoma elimination;treatment group A were added Xing⁃naojing local perfusion on the basis of control group;The treatment group B were used "Tongfukaiqiao" liquor in⁃stilled in rectum on the basis of treatment group A. Results:The total efficiency of treatment group B was 86.0%,which was better than that of treatment group A and control group(P<0.05). The treatment group A and B had more effects than control group in the cerebral hematoma improvements(P<0.05). The treatment group A and B had more effects than control group in the cerebral edema improvements,and effect of the treatment group B is especially obvious. After treatment,the treatment group B and A had more significant differences than con⁃trol group in neurological impairment evaluation(P<0.05);the treatment group B and A had significant differenc⁃es in decrease of neurological impairment evaluation(P<0.05). Conclusion:Tongfukaiqiao liquor of stilling in⁃to rectum on the treatment of hypertension cerebral hemorrhage is significant effect,can be widely clinical used.
