速度向量成像技术评价妊娠期高血压孕妇左心室功能改变的临床研究

摘要

Objective To investigate the clinical value of velocity vector imaging (VVI) in assessment of left ventricular function in women with pregnancy related hypertensive (PRH). Methods 30 women with PRH (PRH group) and 30 pregnant women with normal blood pressure (control group) were enrolled. The regional tissue peak strain (S), systolic peak strain rate (SRs), diastolic peak strain rate (SRd), systolic peak velocity (Vs) and diastolic peak velocity (Vd) of each left ventricular segment were measured on apical four-chamber view by VVI. Results The parameters of S, SRs, SRd, Vs and Vd in left ventricular lateral wall were significantly lower in PRH group than in control group (all P＜0.05). Both Vs and Vd of left ventricular lateral wall gradually decreased from the basal segment to the apical segment in both groups (all P＜0.05). Conclusion VVI is in favour of detecting the influence of pregnancy related hypertension on left ventricular function.%目的 运用速度向量成像技术(VVI)对妊娠期高血压孕妇左心室功能改变进行分析.方法 选择30例被诊断为妊娠期高血压疾病的孕妇作为妊娠期高血压组,另外选择30例无高血压孕妇(正常妊娠组)作为对照.应用VVI在心尖四腔心切面测量左心室各节段峰值应变(S)、收缩期峰值应变率(SRs)、舒张期峰值应变率(SRd)、收缩期峰值运动速度(Vs)及舒张期峰值运动速度(Vd).结果 妊娠期高血压组左心室侧壁及室间隔左心室面S、SRs、SRd、Vs及Vd均较正常妊娠组显著降低,差异有统计学意义(P＜0.05).妊娠期高血压组和正常妊娠组的左心室侧壁及室间隔左心室面Vs和Vd均由基底段至心尖段逐渐减低,差异有统计学意义(P＜0.05).结论 VVI有利于检测妊娠期高血压对左心室功能的影响.
