目的探讨超声心动图组织运动二尖瓣环位移自动追踪技术(TMAD)评价紫杉醇对左心室收缩功能影响的价值。方法52例接受紫杉醇治疗的肿瘤患者,应用超声心动图检测化疗前后左心室舒张末期内径(LVEDd)、左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)和左心室射血分数(LVEF),应用TMAD测量二尖瓣环6个位点的收缩期最大位移(Ds)。结果化疗后与化疗前相比,LVEDd、LVEDV、LVESV增加,差异均有统计学意义(均P<0.05),LVEF减少,但差异无统计学意义(P>0.05)。二尖瓣环6个位点Ds有不同程度减低,其中后间隔位点Ds减低显著(P<0.05)。结论TMAD有助于评价肿瘤患者紫杉醇化疗后左心室收缩功能的变化。%Objective To determine the value of speckle- tracking of mitral annular displacement (TMAD) in evaluating the changes of left ventricular systolic function after paclitaxel chemotherapy. Methods Left ventricular end- diastolic dimension (LVEDd), Left ventricular end- diastolic volume (LVEDV), Left ventricular end- systolic volume (LVESV), and Left ventricular ejection fraction (LVEF) were measured with conventional echocardiography, and the maximal displacement of six points of mitral annulus (Ds) was measured by speckle- tracking echocardiography in 52 cancer patients before and after chemotherapy. Results LVEDd, LVEDV, LVESV increased significantly after than before chemotherapy (P<0.05). LVEF decreased insignificantly. The maximal displacement of six points of mitral annulus decreased accordingly after chemotherapy. The posterior- septal motion significantly decreased (P<0.05). Conclusion TMAD may be benefit to evaluating the left ventricular systolic function change in cancer patients after paclitaxel chemotherapy.
