线性探针技术诊断耐多药肺结核的临床应用评价

     

摘要

Objective To evaluate the clinical effect of linear probe assay (HAIN )on fast diagnosis of multi-drug resistance tuberculosis.Methods A total of 122 patients with smear-positive lung tuberculosis were given HAIN,Lwenstein-Jensen (L-J)cultivation and drug susceptibility test,and the resistance to isoniazid and rifampicin was determined.Results Compared with traditional L-J cultivation,the effect of HAIN on lung tuberculosis patients with multi-drug resistance was not significant (P >0.05).The sensitivity value of HAIN for isonizid and rifampicin was 73.08% and 82.76%,respectively.Moreover,the positive predictive value was 79.17% and 80.00%.The sensitivity and positive predictive value on multi-drug resistance tuberculosis were 57.89% and 64.71%,respective-ly.The specificity and negative predictive value of all results were higher than 90%.Conclusion HAIN is still ef-fective in fast diagnosis of multi-drug resistance tuberculosis,but it shows a few limitations.%目的:评价线性探针杂交技术(简称 HAIN 技术)快速检测耐多药肺结核的临床效果。方法对122例涂阳肺结核患者痰标本同时进行 HAIN 技术和罗氏培养及药敏试验,检测异烟肼、利福平的耐药情况,并对结果进行比较。结果HAIN 技术对肺结核患者耐多药的检测效果和传统罗氏培养法相比无显著差异(P >0.05)。HAIN 技术检测异烟肼和利福平的灵敏度分别为73.08%、82.76%,阳性预测值分别为79.17%、80.00%;检测耐多药的灵敏度为57.89%,阳性预测值为64.71%;所有检测结果的特异度和阴性预测值均>90%。结论HAIN 技术是快速诊断耐多药肺结核的好方法,但检测效果存在一定的局限性。
