超声多普勒评估子痫前期胎儿肺成熟度

     

摘要

目的 探讨超声多普勒评估子痫前期(PE)胎儿肺成熟度的临床应用价值.方法 选取我院自2015年5月-2016年5月间因子痫前期入院分娩的孕妇200例作为研究对象,依据孕周时间分成4组,A组孕周在30-31周间共44例、B组32-33周间共46例、C组34-35周共52例,D组36周共58例,采用超声多普勒测定胎儿主肺动脉血流搏动指数(PI)、阻力指数(RI)情况,评估胎儿肺成熟度,新生儿Apgar评分和新生儿呼吸窘迫综合征(NRDS)发生率;肺成熟度参照羊水泡沫稳定指数.结果 D组胎儿的RI值为(0.84±0.04)、PI值(2.09±0.30)均略低于其他3组,数据对比差异有统计学意义(P<0.05),D组新生儿NRDS发生率为0,Apgar评分为(7.3±0.4)分,与其他3组数据对比差异有统计学意义(P<0.05);D组胎儿羊水泡沫稳定指数为(0.49±0.02),其他3组羊水泡沫稳定指数均<0.47.结论 超声多普勒测定PE胎儿PI、RI值与胎儿肺成熟度密切相关,因此可作为评估胎儿肺成熟度的评估方法.%Objective To discuss clinical application value of Doppler ultrasound on lung maturity of fetal with pre eclampsia ( PE) . Methods 200 pregnant women hospitalized in our hospital from May 2015 to May 2016 were selected as the study objects, and they were divided into 4 groups according to gestational weeks. There were 44 patients in the group A with gestational age of 30-31 weeks, 46 patients in the group B with gestational age of 32-33 weeks, 52 patients in the group C with gestational age of 34-35 weeks, and 58 patients in the group D with gestational age of 36 weeks. Fetal lung maturity index was assessed by ultrasonic Doppler to detect fetal umbilical artery pulsatili-ty index ( PI) and resistance index ( RI) , neonatal Apgar score and the incidence of neonatal respiratory distress syn-drome (NRDS). Pulmonary maturity referred amniotic fluid foam stability index. Results The RI value (0. 84 ± 0. 04) and PI (2. 09 ± 0. 30) in the group D were slightly lower than those in the other groups (P<0. 05). The inci-dence of NRDS in the group D was 0, and the Apgar score was (7. 3 ± 0. 4), which showed significant difference with the other 3 groups (P<0. 05). Amniotic fluid foam stability index was (0. 49 ± 0. 02) in the group D, and the other 3 groups were all lower than 0. 47. Conclusion The value of PI and RI detected by ultrasonic Doppler in PE fetal are closely related with lung maturity, so it can be used as an assessment method to assess lung maturity of fetal.
