节段性或大叶性肺炎支原体肺炎患儿的肺功能变化

     

摘要

目的 探讨节段或大叶性肺炎支原体肺炎患儿肺功能的变化.方法 检测159例节段或大叶性肺炎支原体肺炎患儿在急性期、恢复期的肺功能,并分析比较其变化.结果 节段或大叶性肺炎支原体肺炎患儿急性期的1秒钟用力呼气容积(FEV1)、用力肺活量(FVC)、呼气峰流速(PFF)、25%用力肺活量时的用力呼气流量(FEF25)、50%用力肺活量时的用力呼气流量(FEF50)、75%用力肺活量时的用力呼气流量(FEF75)和最大呼气中期流速( FEF25-75)与预测值比较均明显降低,差异有统计学意义(P<0.01);恢复期各项指标均明显上升,但代表小气道功能的指标FEF50、FEF75、FEF25-75与预测值差异仍有统计学意义(P<0.01).结论 节段或大叶性肺炎支原体肺炎患儿,急性期时大、小气道均受损,恢复期时小气道损伤仍持续存在,定期随访肺功能有利于了解病情恢复情况及预后.%Objective To evaluate the pulmonary function of Mycoplasma pneumoniae pneumonia (MPP) with segmental or lobar inflammation. Methods Pulmonary function was tested in 159 children of MPP with segmental or lobar inflammation during the acute phase and recovery phase, and the changes of pulmonary function were analyzed and compared. Results In acute stage of MPP with segmental or lobar inflammation, forced expiratory volume in first second (FEV,) , forced vital capacity (FVC) , peak expiratory flow (PEF) , 25% forced expiratory flow (FEF25) , 50% forced expiratory flow ( FEF50) , 75% forced expiratory flow ( FEF75) and forced expiratory flow between 25 and 75% of FVC (FEF25_75) were decreased obviously. Compared with the predictive value, the differences were statistically significant (P < 0.01). In recovery phase, these indicators were significantly increased and compared with the predicted values, FEF50, FEF75 and FEF25-75 which reflect the small airway function were still statistically significant (P < 0.01 ). Conclusions In acute phase, the large and small airways are all damaged in children of MPP with segmental or lobar inflammation. In recovery phase, the injury of small airway is persisted. The testing and following-up of pulmonary function would help deepen our understanding towards to patients' condition and prognosis.
