频域光学相干断层成像术检测黄斑区视网膜神经上皮容积的初步研究

             

摘要

目的 应用频域光学相干断层成像术(OCT)测量正常成年人黄斑区视网膜神经上皮层容积,分析黄斑区视网膜内层三维立体结构特征,为评价黄斑区视网膜疾病的神经上皮层容积改变提供参考数据.方法 303名健康人(303只眼)经视力、眼压、裂隙灯、屈光和眼底等常规眼科检查,排除黄斑病变、青光眼、视神经和视网膜病变等影响黄斑区视网膜厚度的疾患后,行双眼以黄斑为中心的三维光学相干断层扫描.每名受检者随机抽取一眼的测量结果进行统计.结果正常人黄斑中心凹1 mm直径范围的容积最小(0.175±0.015) mm3,黄斑区内环(颞、上、鼻、下)各象限平均容积分别为(0.450±0.025) mm3、(0.463±0.025) mm3、(0.475±0.027) mm3、(0.463±0.025) mm3,黄斑区外环(颞、上、鼻、下)各象限平均容积分别为(1.284±0.072) mm3、(1.316±0.069) mm3、(1.463±0.087) mm3、(1.312±0.075) mm3;不同性别、眼别之间黄斑区各象限容积无显著性差异;除60岁以上年龄组外,黄斑区各象限容积均不随年龄增加而改变,而60岁以上年龄组各象限容积较60岁以下各年龄组偏小,差异有统计学意义.结论 频域OCT能够精确地测量黄斑区视网膜神经上皮层容积,本研究初步建立正常成年人黄斑区视网膜神经上皮容积数据库,为今后临床和科研提供参考.%Objective To measure macular neurosensory retinal volume in normal adult Chinese using frequencydomain optical coherence tomography( FD-OCT ). Methods 303 healthy Chinese ( 303 eyes ) participants underwent a general ophthalmic examination,including measurement of the visual acuity .intraocular pressure and dioptroscopy, fundus examination and slitlamp examination. None of participants had macular disease, glaucoma, retinopathy or disease of the optical nerve and retina. Three-dimensional scan of the macula were performed in one eye of all participants by OCT( 3 D OCT-IOOO mark Ⅱ; Topcon ). Data for one randomly selected eye of each subject were used for statistical analysis. Results The fovea macular retinal volume was the smallest( 0. 175 ±0. 015 mm3 ). There were no gender-related differences in macular neurosensory retinal volume. There were age-related differences in the people aged over sixty, but no statistical differences in the people aged under sixty. Conclusion Macular neurosensory retinal volume was determined by FD-OCT for a normal Chinese population. The normative data from this study may offer valuahle information for future studies of ocular macula and comparisons between different populations.
