地特胰岛素或中效鱼精蛋白锌胰岛素对血糖控制不佳的2型糖尿病患者的影响

Objective To observe the effects of additional administration of insulin detemir or neutral protamine hagedom (NPH) on type 2 diabetic patients with poor glucose control.Methods Thirty-six type 2 diabetic patients with poor glucose control after oral administration of hypoglycemic drugs were randomly divided into insulin detemir group and NPH group,and the patients in 2 groups were respectively administrated insulin detemir and NPH before going to bed for 12 weeks.Results After treatment,the levels of fasting blood glucose (FBG) 2 h blood glucose (2 hBG) and glycosylated hemoglobin (HbAlc) in both groups decreased markedly,and those in NPH group slightly lower than insulin detemir group,but the difference showed no statistical significance.The incidence of hypoglycemic events in insulin detemir group was notably lower than that in NPH group,and the increased body mass slightly lower than that in NPH group.Conclusion Additional administration of insulin detemir and NPH to type 2 diabetic patients with poor glucose control can effectively reduce blood glucose,and both of them have similar therapeutic effects.However,insulin detemir can control the body mass of patients and decrease the incidence of hypoglycemia.%目的 观察血糖控制不佳的2型糖尿病患者加用地特胰岛素或中效鱼精蛋白锌胰岛素(NPH)对血糖的影响.方法 将36例口服降糖药血糖控制不佳的2型糖尿病患者随机分为地特胰岛素组和NPH组,2组患者睡前分别加用地特胰岛素和NPH,治疗12周.结果 治疗后,2组患者空腹血糖(FBG)、2h血糖(2hBG)和糖化血红蛋白(HbAlc)均较治疗前明显下降,NPH组FBG、2hBG及HbA1c水平略低于地特胰岛素组,但差异无统计学意义;地特胰岛素组低血糖事件发生率显著低于NPH组,且体质量增加略低于NPH组.结论 对血糖控制不佳的2型糖尿病患者加用地特胰岛素或NPH均能有效降低血糖,且2者疗效相当,但地特胰岛素比NPH更能控制患者体质量,减少低血糖的发生.
