手术与手法复位治疗Weber-DenisC型踝关节骨折的临床疗效分析

Objective ToexploretheclinicalefficacyofmanipulationvsoperationintreatmentofpatientswithWeber-DenisCankle fractures.Methods Atotalof125patientswithWeber-DenisCanklefractureswererecruitedinthisprospectivestudywithoperationreduction groups 61 cases and manipulation reduction group 64 cases respectively. The results of recovery of ankle function in these 2 groups were evaluated afterfollow-upfor1year.Results Onehundredandtwentypatientscompletedfollow-upwerefinallyanalyzedinthisstudy.Thelengthof hospitalization stay in operation reduction group was longer than that of manipulation reduction group with statistical significance( P ﹤0. 05). The healing time in operation reduction group was shorter than that of manipulation reduction group( P ﹤0. 05). The fineness rate in operation reduc-tiongroupwas87.9%whichwasmuchhigherthanthatofmanipulationreductiongroup(67.7%)accordingtoMazur'sscores.Conclusion Pa-tients in operation reduction group have longer hospitalization stay but shorter in healing time and superior fineness rate compared to those of manip-ulation reduction group. Operation is recommended for treatment of patients with Weber-Denis C ankle fractures in good condition.%目的：探讨手术与手法复位治疗Weber-DenisC型踝关节骨折的临床疗效。方法 C型踝关节骨折患者125例，随机分为手术治疗组61例，手法复位组64例。两组术后随访1年，对患者术后踝关节功能恢复情况进行评价。结果共120例完成随访，手术组58例，手法组62例。手术组患者住院时间长于手法组，但手术组骨愈合时间短于手法组，差异有统计学意义（ P ﹤0.05）。手术治疗组总优良率为87.9%，明显高于手法组的67.7%（ P ﹤0.05）。结论手术治疗C型踝关节骨折虽住院时间长，但是在骨愈合时间和踝关节功能恢复方面明显优于手法复位组，对于全身情况较好的C型踝关节骨折患者应尽量采用手术治疗。
