目的：探讨完整结肠系膜切除（ CME）在结肠癌治疗中的价值。方法将2012年12月至2013年12月采用CME治疗的结肠癌患者35例作为观察组，将2011年3月至2012年4月采用传统根治术治疗的结肠癌患者35例作为对照组。评估两组手术质量，对比淋巴结清扫数量、术中平均出血量、平均手术时间以及术后排气时间，观察两组术后感染、吻合口瘘等并发症以及复发情况。结果观察组手术质量C级占100％，明显高于对照组（51.43％）；观察组淋巴结平均清扫数目为23.5±4.6枚，高于对照组（18.4±2.3枚），观察组手术时间与对照组无显著差异，但出血量较对照组少，术后排气较对照组早，观察组并发症发生率为8.57％，明显低于对照组（20.0％），且复发率（5.71％）也明显低于对照组（17.14％）。组间比较差异均有统计学意义（ P <0.05）。结论 CME治疗结肠癌临床疗效明显，较传统结肠癌根治术效果好，且并发症发生率和复发率低。%Objective To explore the value of a complete mesocolon resection( CME)in the treatment of colon cancer. Methods Between December 2012 and December 2012,35 cases of colon cancer patients treated with CME were selected as observation group. Between March 2011 to April 2012,the traditional radical treatment were performed in 35 cases of colon cancer patients,which were considered as control group. These inde-xes were analyzed and compared including the lymph node evaluation,operation quality and quantity,intraoperative blood loss,average operation time,postoperative exhaust time,postoperative infection,complications such as anastomotic fistula,and relapse. Results The rate of grade C in the observation group accounted for 100%,which was significantly higher than the control group(51. 43%). The average lymph node cleaning the num-ber of the observation group was 23. 5 ± 4. 6,which was higher than control group(18. 4 ± 2. 3). The operation time of observation and control group had no significant difference,but blood loss less than the control group. Postoperative exhaust earlier than the control group. The complication rate of observation group was 8. 57%,which is significantly lower than the control group(20. 0%). The recurrence rate(5. 71 %)is significantly,which is lower than the control group(17. 14%). Compare the differences between groups were statistically significant( P <0. 05). Conclusion The effect of CME in colon cancer patients is safety and feasible. This method had better effect comparing the traditional method.
