腹腔镜手术联合 GnRH-a 促性腺激素治疗卵巢巧克力囊肿的临床疗效及对患者卵巢功能的影响

Objective To investigate the clinical effect of laparoscopic operation combined with gonadotropin in the treatment of ovarian chocolate cyst and its effect on ovarian function. Methods 82 patients with ovarian chocolate cyst in our hospital from June 2011 to June 2013 were randomly selected as the research objects. The 41 patients treated with laparoscopic surgery were treated as the control group. The clinical effect,recurrence rate and pregnancy rate were compared between the two groups. Results The total effective rate was 97. 56%(40 / 41)in the control group,the difference was statistically significant( P < 0. 05),the recurrence rate was 2. 44%(1 / 41),the recurrence rate was 21. 95%(9 / 41),the difference was statistically significant( P < 0. 05). The difference was statistically significant( P < 0. 05). The 2 year pregnancy rate after operation was 46. 34%(19 / 41),and the difference was statistically significant( P < 0. 05). Conclusion The clinical effect of lapa-roscopic surgery combined with gonadotropin treatment in patients with ovarian chocolate cyst is remarkable,and it can improve the patient′s symp-toms,reduce the recurrence rate of the cyst,and has good clinical application value.%目的：探讨腹腔镜手术联合 GnRH - a 促性腺激素治疗卵巢巧克力囊肿的临床疗效及对患者卵巢功能的影响。方法随机抽取自2011年6月至2013年6月期间收治的卵巢巧克力囊肿患者82例患者作为研究对象，按治疗方法的不同，将单纯接受腹腔镜手术治疗的41例患者作为对照组，将接受腹腔镜手术联合促性腺激素治疗的41例患者作为观察组，观察对比两组患者的临床疗效和治疗前后的性激素水平，术后随访2年，对比其复发率及妊娠率。结果观察组总有效率为97.56%（40/41），明显高于对照组总有效率78.05%（32/41），差异有统计学意义（ P <0.05）；观察组复发率为2.44%（1/41），明显低于对照组复发率21.95%（9/41），差异有统计学意义（ P <0.05）；两组患者术后 E2、P、FSH、LH 水平较术前均有改善，差异有统计学意义（ P <0.05）；观察组术后雌二醇（E2）、孕激素（P）、促卵泡素（FSH）、促黄体素（LH）水平优于对照组，差异有统计学意义（ P <0.05）；观察组术后2年妊娠率为46.34%（19/41），明显高于对照组术后2年妊娠率为21.95%（9/41），差异有统计学意义（ P <0.05）。结论对卵巢巧克力囊肿患者采取腹腔镜手术联合 GnRH - a 促性腺激素治疗的临床效果显著，改善患者的性激素水平，降低术后囊肿复发率，在临床中具有良好的应用价值。
