腹腔引流导管用于胸腔闭式引流治疗胸腔积液的临床疗效分析

摘要

Objective To observe and compare the efficacy of closed drainage of pleural cavity by disposable central venous catheter for treatment of patients with pleural effusion, and to provide basis for the promotion of catheter for abdominal cavity drainage for pleural closed drainage in treatment of patients with pleural effusion.Methods A total of 171 patients with pleural effusion in this hospital during March 2014 to March 2015 as research objects (108 males and 63 females).All these patients were randomly divided into observation group and control group.Patients in control group (83 cases) were treated by disposable central venous catheter, and patients in observation group (88 cases) were treated with abdominal cavity drainage catheter.The difference in indexes as curative effect, postoperative complications, duration for drainage, operating time and duration for hospitalization between these two groups had been observed during the course of treatment.Results In comparison with control group, the curative effect of observation group was better, and the rate of subcutaneous emphysema and blocking pipe were significantly less than those of control group.Besides, there was no significant difference in several other indicators between these two groups.Conclusion The results showed that abdominal cavity drainage catheter group have better effect in treatment of pleural effusion compared with that of central venous catheter group.However, the abdominal cavity drainage catheter group showed the low rate of occurrence of tube plugging and subcutaneous emphysema.%目的 比较腹腔引流导管进行胸腔闭式引流术与一次性使用中心静脉导管胸腔闭式引流术在治疗胸腔积液病人的疗效,为推广腹腔引流导管进行胸腔闭式引流术治疗胸腔积液提供依据.方法 以2014年3月至2016年12月就诊的171例胸腔积液患者为研究对象.将其随机分为观察组和对照组,对照组83例,采用一次性使用中心静脉导管进行胸腔闭式引流;观察组88例,采用腹腔引流导管进行胸腔闭式引流.对比观察两组的临床疗效、并发症、引流时间、手术时间及住院时间.结果 与对照组比较,观察组显效率(85.2%)和总有效率(96.6%)明显高于对照组(74.7%,89.2%),而且皮下气肿数(0例)和堵管数(2例)明显少于对照组(3例,6例),差异均有显著性(P0.05).结论 腹腔引流导管作胸腔闭式引流术在治疗胸腔积液方面临床疗效优于中心静脉导管,而且具有堵管率低、皮下气肿发生率低等优势.
