后路微创钉棒系统治疗胸腰椎骨折43例的临床疗效分析

摘要

目的 对比分析后路经皮微创钉棒系统与传统切开复位内固定术治疗胸腰椎骨折患者的临床疗效.方法 选择2014年3月至2014年12月收治的43例患者,所有病例均为单节段胸腰椎新发骨折,随机分为微创组(n=24)和传统组(n=19).微创组采用德国BRAUN微创后路S4钉棒系统内固定术,传统组采用传统后路切开复位内固定术,两种术式均为单节段跨伤椎固定.观察比较两组手术相关指标及影像学数据.结果 微创组Macnab评分优良率(83.33%)显著高于传统组(73.68%,p0.05).结论 后路经皮微创钉棒系统内固定术治疗胸腰椎骨折的临床疗效优于传统手术,在恢复胸腰椎生理曲度方面与开放手术相当,可以有效改善患者的生活质量,值得临床推广.%Objective To investigate and compare the clinical efficacy of posterior minimally invasive percutaneous nail rod fixation system and traditional open operation in the treatment of thoracolumbar fractures.Methods 43 cases of thoracolumbar fracture patients in our hospital were randomly divided into minimally invasive group (MIG, n=24) and open group (OG, n=19) from March 2014 to December 2014.All cases were single segment thoracolumbar fractures without neurological symptoms.The OG was treated by traditional open surgery.The MIG was treated by posterior minimally invasive percutaneous nail rod fixation system (BRAUN in Germany).Both of them were performed single segment of vertebral fixation.The operation related indexes and imaging data of two groups were compared.Results The incision length, operation time, bleeding volume, postoperative drainage, VAS score of postoperative incision pain and score of Macnab in MIG were lower than those in OG with significant differences (p0.05).After treatment both in MIG and OG, the loss percentage of anterior vertebral height and cobb angle in sagittal plane had a statistical significant (p0.05).Conclusion The posterior minimally invasive percutaneous nail rod fixation system has a better clinical efficacy than traditional open operation in the treatment of single segment thoracolumbar fractures.The recovery of thoracic and lumbar lordosis in both methods are equal.It can effectively improve the quality of life, worth of wide clinical application.
