不同留针时间对缺血性脑卒中后疲劳患者生活质量的影响

摘要

Objective:To observe the effect of needle retention time on fatigue of post ischemic stroke in pa-tients.Methods:The patients ( n =80 ) were randomly divided into a 30 -min group and a 60 -min group ( each n=40) by using computer software to get the random numbers.One case was shedded in the 60-min group.Two groups were both given the same acupuncture point selection once a day, acupoints group one:WANG Le-ting hand-foot -12 -needle, acupoints group two: WANG Le-ting curing Back -shu.Two groups were used interchangeably,five times a week, one course for 4 weeks.The needle retention times were 30 minutes in 30-min group and 60 minutes in 60-min group.The scores were recorded by applying the US National Institutes of Health Stroke Scale(NIHSS),Fatigue Assessment index (FAI),Stroke-specific quality of life scale( SS-QOL) respectively before treatment and after treatment for one course.Results:After treatment for one course, the total scores of NIHSS, FAI and SS-QOL had a significant difference in each group before treatment and after treatment (P <0.01).The total scores of NIHSS,FAI and SS -QOL had a significant difference between two groups after treatment ( P<0.05 ) .Conclusion:The 30 minutes needle retention time can achieve a satisfactory result for improving the symptoms of fatigue and neurological deficit, thus improving the quality of life in patients.%目的：观察不同留针时间对缺血性脑卒中后疲劳患者生活质量的影响。方法：将80例缺血性脑卒中疲劳患者按计算机软件所获取的随机号，随机分为30 min组和60 min 组，每组各40例，留针60 min组脱落1例。两组在一般治疗的基础上，给予针刺治疗，取穴：1组：王乐亭手足十二针—曲池、合谷、内关、足三里、阳陵泉、三阴交；2组：王乐亭治背俞穴—五脏俞加膈俞。两组穴位隔日交替选用，每穴均行平补平泻的提插捻转手法，以得气为度。留针30 min组，针刺得气后留针30 min；留针60 min组，针刺得气后留针60 min。两组患者均每日针刺1次，每周治疗5次，4周为一疗程。分别在治疗前、治疗1个疗程后，采用美国国立卫生研究院卒中量表（ NIHSS）、疲劳评定量表（ FAI）、脑卒中专用生活质量量表（ SS－QOL）对患者进行评定。结果：两组治疗后NIHSS 、FAI、SS－QOL评分值较同组治疗前均有明显改善（P＜0．01）；两组治疗后组间比较具有显著性差异（P＜0．05）。结论：留针30 min较留针60 min能更有效改善脑卒中后患者疲劳症状、神经功能缺损缺症状，提高患者的生活质量。
