Inhibin、CD99、Calretinin、WT1和Melan-A在卵巢粒层细胞瘤中的免疫组织化学表达

         

摘要

Objective Immunohistochemical markers were determined in GCT,and the distinction of GCT and SLCT was discussed together with clinical data and histomorphology. Methods Twenty-two cases of GCTs and 13 cases of SLCTs were selected. Five common markers of the ovarian sex cord stromal tumors(including Inhibin,CD99,calretinin,WT1 and melan-A)and many other non-specific markers(including CK5/6,CK18,EMA,Vimentin,SMA, S-100,CD10,ER,PR,AR,p53,Ki67)were determined immunohistochemically. Results Inhibin and WT1 represtented the highest and lowest expression rate(94%and 78%,respectively)in the granulosa cells of GCT,meanwhile,WT-1 and CD99 showed the highest and lowest expression rate(100%and negative,respectively)in the stromal cells of GCT. Conclusion A panel of immunohistochemical markers including Inhibin,CD99,calretinin,WT1 and melan-A together with clinical data and histomorphology was very helpful for the differential diagnosis of GCT and SLCT.%目的:检测多种免疫组织化学标记物在卵巢粒层细胞瘤(GCT)的表达情况,结合临床资料及组织形态学特征讨论此瘤与卵巢支持-间质细胞瘤(SLCT)的鉴别诊断。方法收集22例GCT(成年型18例,幼年型4例)与13例SLCT(高分化6例,中分化7例),检测5种常用的卵巢性索间质肿瘤免疫组织化学标记物(包括Inhibin、CD99、calretinin、WT1和melan-A)以及其他多种非特异性标记物(包括CK5/6、CK18、EMA、Vimentin、SMA、S-100、CD10、ER、PR、AR、p53、Ki67)在这两种肿瘤中的表达特征。结果在GCT的粒层细胞,Inhibin的表达率最高(94%),而WT1的表达率最低(78%);而在GCT的间质细胞,WT1的表达率最高(100%),CD99的表达率最低(阴性)。其它非特异性标记物在GCT和SLCT中表达的敏感性和特异性均较低。结论结合临床特征与组织学特点,用Inhibin、CD99、calretinin、WT1和melan-A作为一组抗体,有助于GCT与SLCT的鉴别诊断。
