脑静脉性血管畸形的CT和MRI诊断

             

摘要

Objective To explore the diagnostic value of CT,conventional MRI and susceptibility weighted imaging(SWI) in cerebral venous malformation ( CVM). Methods Imaging data of 34 patients with CVM were analyzed retrospectively. 10 patients had CT plain scan,32 patients had conventional MRI,and 11 cases were imaged with Gd-DTPA enhanced MRI, 18 cases had SWI examination. Results CT plain scan showed a few drain veins. Conventional MRI showed many drain veins and some medullary veins. Contrast-enhanced MRI and SWI showed all of the veins and typical caput medusae sign. Conclusion The role of CT plain scan in diagnosis of CVM is restricted. Conventional MRI and Contrast-enhanced MRI are effective. SWI is of high sensitivity and specificity for the detection of CVM with associated cryptic vascular malformation without use of contrast material. It should be the first choice for the diagnosis of CVM.%目的 探讨CT、常规MRI及磁敏感加权成像(SWI)技术在脑静脉性血管畸形中的诊断价值.方法 对34例静脉性血管畸形患者的影像资料进行回顾性分析.其中10例患者行CT平扫,32例行常规MRI平扫,11例行Gd-DTPA增强扫描,18例行SWI检查.结果 CT平扫可显示部分扩张的引流静脉,MRI平扫可显示大部分扩张的引流静脉及部分扩张的髓静脉.MRI增强扫描及SWI可显示全部的引流静脉及扩张的髓静脉,呈典型“水母头”样改变.结论 CT平扫对诊断颅内静脉畸形的价值有限,而MRI平扫及增强扫描对静脉畸形具有较高的诊断价值.SWI无需注射对比剂,能够清晰地显示静脉畸形以及伴发的隐匿性血管畸形,可以作为脑静脉性血管畸形首选的检查方法.
