The present invention relates to a real-time alignment system for aligning an X-ray angiography image and a CT angiography image, and a method for conducting a cardiovascular operation by aligning a CT angiography image and an X-ray angiography image. According to the present invention, since images are aligned using a radiopaque hemispherical three-dimensional marker, it is possible to rapidly respond to all the external environments, such as patient's breathing or movement, and to enable real-time treatment guidance in the site of conducting an operation, thereby maximizing accuracy of the operation.
