NSE、D-D、CRP在重型颅脑损伤病情判断中的价值

         

摘要

目的 探讨神经元特异性烯醇化酶(NSE)、D-二聚体(D-D)、C-反应蛋白(CRP)在重型颅脑损伤病情判断中的价值.方法 选取2014年1月至2017年1月本院收治重型颅脑损伤患者100例作为观察组,另选取同期在本院进行体检的健康志愿者35例作为对照组.检测患者血清中NSE、D-D及CRP水平,分析其在不同临床特征的意义及对预后判断的价值.结果 入院时,危重组患者血清NSE、D-D及CRP水平高于观察组和对照组患者,观察组血清NSE、D-D及CRP水平高于对照组患者(P<0.05).不同类型脑损伤患者血清NSE、D-D及CRP水平比较无统计学差异(P>0.05).观察组患者入院第3天血清NSE和CRP水平达到最高峰,入院第3天后NSE和CRP水平开始下降(P<0.05).观察组患者入院第1天血清D-D水平最高,随着时间延长,D-D水平逐渐下降(P<0.05).死亡组患者血清NSE、D-D及CRP水平高于存活组患者(P<0.05).结论 观察组者血清NSE、D-D及CRP水平与疾病的严重程度密切相关,可作为判定颅脑损伤的指标,具有临床应用价值.%Objective To investigate the value of neuron-specific enolase (NSE), D-two (D-D) and C-reactive protein (CRP) in the diagnosis of severe traumatic brain injury.Method From January 2014 to January 2017 in our hospital one hundred cases of severe craniocerebral injury patients were selected as the observation group, were examined in our hospital thirty-five cases of healthy volunteers as control group.Detection of serum NSE, D-D and CRP levels, the analysis on the prognosis in different clinical features and the significance of the value.Results At admission, the levels of serum NSE, D-D and CRP in patients with severe recombinant were higher than those in the severe group and the control group, and the serum levels of NSE, D-D and CRP in the severe group were higher than those in the control group (P<0.05). Different types of brain injury in patients with NSE, there was no statistical difference between D-D and CRP (P>0.05). Observation of NSE and CRP levels in serum of patients reached a peak of third, third days after admission NSE and CRP level began to decline (P<0.05).In the observation group, the serum level of D-D was highest in first days after admission, and the level of D-D decreased gradually with the prolongation of time (P<0.05).The death of serum NSE, D-D and CRP levels higher than the survival of patients (P<0.05). Conclusion The serum NSE in patients with severe craniocerebral injury, closely related to the severity of D-D and CRP levels and the disease, can be used to predict the brain damage index has clinical application value.
